**Job Summary**:

Reporting to the Director of Family and Community Health, the Manager of Outreach in Public Health is responsible for providing strategic direction and leadership to a team of professionals to ensure effective and efficient planning, implementation, and evaluation of public health outreach initiatives and interventions to improve the health and health equity of the community, and in accordance with departmental and legal mandates, and as outline in the Ontario Public Health Standards mandate. This role involves coordinating with various internal and external stakeholders, including healthcare providers, community organizations, and government agencies, to promote and implement health programs and services and target priority populations to reduce health disparities and improve the overall health of Niagara residents.

**Education**:

- Post secondary degree in Nursing, health sciences, public health, or related discipline
- Masters degree in Health Sciences, Public Health, or administration

**Knowledge**:

- 3 years experience in public health, public health outreach, and community engagement
- 5 years experience in management/supervisory capacity
- Advanced knowledge of public health issues, community resources, priority populations, community engagement principles, and the social determinants of health

**Responsibilities**:

- Plans, implements, monitors and evaluates the delivery of public health outreach programs, interventions, and initiatives in compliance with the Ontario Public Health Standards and identified local needs (30% of time)_
- Determines program direction, goals and objectives; establishes priorities utilizing evidence-based practice, available data and community needs assessments; and monitors and evaluates program and intervention outcomes
- Develops annual business plans that align with the Corporation's Business Plan, Niagara Region Public Health & Emergency Services strategic direction, and the Ontario Public Health Standards
- Makes business decisions based on an accurate understanding of program, intervention and population health data.
- Develop and implement outreach strategies and programs to increase awareness and engagement in public health initiatives.
- Determines opportunities for continuous quality improvement initiatives, ensuring resource availability.
- Facilitates the uptake of evaluation results and continuous quality improvement recommendations
- Participates on internal and/or external committees and task forces, attending meetings to receive information, plan strategies and disseminate information to appropriate work groups
- Develops reports for the Director, Medical Officer of Health, and the Ministry Of Health, as required
- Develops and updates policies and procedures
- Initiates research opportunities and/or prepares funding proposals, where appropriate
- Understands and applies data governance policies, procedures, and principles to maintain the integrity and security of sensitive and confidential data, including personal health information

**_ Liaises with community partners and agencies, primary care providers, and other public and private sector partners and volunteers, as appropriate. (20% of time)_**:
