**Job Summary**:
Salary Pending Review

Reporting to the Manager of Outreach, the Outreach Supervisor will lead and manage outreach initiatives and interventions to promote health education and services within the community. This role involves supervising outreach staff, developing and implementing outreach strategies, and collaborating with community organizations to address public health issues and improve community health outcomes in accordance with departmental and legal mandates and as outlined in the Ontario Public Health Standards mandate.

**Education**:

- Post secondary degree in Nursing, Health Sciences, Public Health or health related discipline
- Masters in Health Sciences, Public Health or administration preferred

**Knowledge**:

- 3 years experience working in a clinic setting or health care setting
- 3 years experience in public health outreach
- Previous supervisory experience is preferred
- Advanced knowledge of public health issues, community resources, and outreach strategies
- Ability to work effectively with diverse populations and build strong community partnerships

**Responsibilities**:

- Supervises staff, coordinating and planning work, providing work direction, assigning tasks and projects, determining methods and procedures to be used, monitoring quality of work, resolving problems and ensuring assigned results are achieved. (40% of time)_
- Enables results with the organization’s human capital strategy to foster employee engagement
- In conjunction with the Manager provides training and development, coaching and discipline when necessary, and manages staff recruitment
- Ensures focus is service excellence, communication/transparency, innovation, and data integrity and work flow integration
- Ensures staff has the information and resources to make successful plans and decisions
- Ensures all people related issues, including recruitment, grievances and labour relations issues, are aligned to HR and Corporate standards and practices
- Helps to break down barriers to employee success, ensuring collaboration and cooperation with other teams within their division and department
- Ensures Occupational Health & Safety policies, programs and practices are implemented, and maintained. This includes workplace inspections, monitoring, accident reporting and investigations, and ensuring any observed hazards or lapses in the functioning of OH&S processes, and other OH&S concerns are responded to promptly
- Ensures all individuals under supervision have been informed of hazards and instructed on the necessary risk control and emergency response measures
- Supports the Manager with the development, maintenance and evaluation of all program services planning and coordinating program services: (20% of time)_
- Perform audits to ensures client records are accurate, complete and appropriate confidentiality is maintained
- Provides direct oversight, consultation and support of staff on the outreach team
- Monitors workload management and provides regular reports to the Manager
- Completes and interprets statistics/data for reports required by the Manager, Division Director and Medical Officer of Health/Associate Medical Officer of Health
