Reporting to Director, Capacity, Access & Flow, the Lead, Capacity, Access & Flow (Long Term Care & Post Acute) is responsible for implementing and monitoring initiatives that support the broad health care system across the West Region. Working collaboratively with members of this and other Ontario Health (OH) West Region portfolios, the Lead will contribute to regional capacity planning, enhancing access to care and improving patient flow. More specifically, this role will provide leadership and support to activities such as those related to monitoring and implementing improvements to Long Term Care (LTC) placement and occupancy as required, in collaboration with LTC homes, Home and Community Care Support Services, and Ministry of Long Term Care; monitoring and tracking LTC home redevelopment functions, including informing capacity planning activities, and optimizing LTCH and post acute (rehabilitation and complex continuing care) service utilization. Supporting COVID-19 response activities related to surge capacity and load levelling, as well as assistance with system recovery and transformation will be required. The role will support LTC Home capacity planning and acute/post acute and community identified services capacity planning across West Region to identify and mitigate access gaps, in addition to supporting planning work related to recommendations emerging from LTC commissions, reviews and policy that relate to OH regional role.

The Lead, Capacity, Access and Flow (Long Term Care and Post Acute) will serve as a main contact to coordinate system level work within a defined area of focus and/or a defined geography. The Lead will also be responsible for conducting analysis of the local health system to identify population and service needs, identify and resolve barriers to access and patient flow. The Lead will identify and foster West Region relationships among health service providers and related sectors that will result in improving the health of the population, enhancing the experience of care, and sustaining integrated service delivery.

Here is what you will be doing:
- Coordinates, plans, and supports implementation of initiatives that improve health system capacity, access and patient flow, specific to LTC and acute/post acute work- Monitors, analyzes and/or interprets local health-care system information based on provincial data, community input and other sources for purpose of identifying barriers and opportunities to improve capacity, access and flow.- Leads quality improvement and/or implementation activities such as those related to LTC placement and occupancy, LTCH and acute/post acute capacity planning- Participates at sub-regional committees as required.- Conducts effective stakeholder consultation related to planning, assesses impact of planning recommendations and decisions, and promotes consensus among stakeholders for action required.- Performs high-quality planning, research and analysis of local and regional data, community input and other sources in order to identify gaps and provide input to planning- Provides advice and input from the planning perspective to the development of health-care system funding plans.- Collaborates with other OH staff on priorities, work plans, project and performance monitoring- Participates in stakeholder engagement and provides analysis of information and data necessary to foster effective discussion and inform initiatives and project plans.- Prepares briefing documents and reports related to OH plans, initiatives and projects.- Stays abreast of policy and legislation, best practices and methodologies in health-care planning and service delivery in order to identify and recommend best planning approaches and opportunities.- Actively participates within cross-functional teams to advance the OH's strategic directions
