Bilingual Team Lead - Coordination and Access

Franco-Santé London( AFSL). **The innovative project aims to facilitate the offer of French services to the region’s francophone community and improve access to information, programs and services related to health and well-being**
**This project, spearheaded by** Accès Franco-Santé London, **originated from a partnership with the** London
Inter Community Health Centre. Accès Franco-Santé London offers the francophone community a central point of access to health and social services and resources in French, in a comprehensive and coordinated manner, to enable them effectively to navigate the healthcare system.
**Permanent full-time position**- 37.5 hours per week
**Location - **920 Huron St, London, ON
**Annual salary **- $60,000 to $62,000 plus benefits
**Start date **- As soon as possible.
**Position Summary**
As the Team Lead, Coordination and Access, you will be instrumental to the Hub’s success. You will coordinate the AFSL’s Day to day activities as well as lead the development of the standards, procedures and plans that will drive how the Hub will achieve its goal of facilitating French speaking community members’ access to programs, services, resources, and information related to their health and well-being.
Your entrepreneurial, innovative spirit and collaborative approach will enable you to connect with community members, partners, and service providers to improve the experience of francophone community members in accessing health and social services resources in French. You will leverage your communication and leadership skills to engage stakeholders around a common vision and foster partnerships that ensure that AFSL achieves its goal of improving French speaking community members’ ability to navigate the health system and seamlessly accessing health, mental health and addictions, social and wellbeing services.
**Key Responsibilities**
**Development, Implementation and Evaluation**
In collaboration with the Hub’s steering committee:
- Lead the development of various plans, processes, and protocols to inform how the Hub delivers its programs and services, including the development and implementation of the Hub’s standard operating procedures, operational plan, outreach strategy and communications plan.
- Assist with the development of program policies, procedures, guidelines, standards, and initiatives at the Hub and make recommendations to the Steering Committee and Partners for changes and/or improvements.
- Propose key performance indicators to track and monitor the AFSL’s effectiveness.
- Systematically evaluate the quality and effectiveness of Hub services, analyzing appropriate internal and external data and information to identify opportunities in collaboration with interested parties for improving services and client outcomes.
- Monitor service pressures including service levels, waitlists, etc. and ensure the provision of safe, high-quality and timely services that are client and community centered.
- Prepare and submit monthly reports tracking progress against desired results.
**Program and Service Delivery**
- Ensure partners are fulfilling their commitments to the Hub (AFSL) as per the terms of reference.
- Promote and facilitate effective, positive communication and staff dynamics
- Mentor positive behaviour including open communication, respect, professionalism and serving as a positive role model to Hub’s environment.
- Develop, monitor and coordinate daily staffing requirements, to ensure adequate coverage.
- Provide orientation / training to new partners, staff, and volunteers.
- Responsibility for day-to-day operation and performance review activities with Hub partners.
- Identify and investigate possible opportunities and partnerships that enhance services and operations; contribute to the development of program proposals to support these initiatives.
- Seek out and maintain knowledge of current best practices in the field, sharing information with staff and evaluating programs in relation to those best practices on a regular basis.
- Support the development of competencies and skills in staff to perform the duties of the role in partnership with staff’s agency.
- Coordinate the delivery of health promotion workshops (e.g., mental health and addiction, flu shot clinics, physical exercise, etc.).
**Community Engagement**:
- Advocate, on behalf of clients and community, to ensure that community supports, health and social services, especially mental health services, are equitably accessible and high quality in French.
- Develop new partnerships and strengthen existing relationships with Hub partners.
