**Duties and Responsibilities Include**:
- Perform all duties in accordance with the Mission Statement, Philosophy and Objectives of St. Joseph’s Villa and the standards and strategic plan of the Nursing Department.
- Provides clinical services to clients including admission assessments, consents, care planning and admission support to resident and family.
- Organization of annual and post admission reviews.
- Understanding of the Long Term Care Homes Act and Regulations.
- Collaborate with community resources
- hospital, LHIN partnerships.
- Review of intake files for potential admission and waitlist through the online portal.
- Liaison for community support service agencies and resources.
- Coordinate and implement intake process.
- Coordinate and lead the Admission, Discharge and Transfer Committee team meetings.
- Attend LHIN meetings.
- Other duties as assigned
**Education and Qualifications**:
- Experience in Nursing is required
- RPN certificate is required. Must be registered with the CNO in good standing
- Community support service sector experience is preferred
- Knowledge, skill, and sensitivity of Long-Term Care and working amongst the elderly is preferred
- Knowledge of current developmental and research in Gerontology is an asset
- Demonstrated leadership ability is preferred
- Effective communication skills to deal with the public and community agencies is an asset
