Reporting to the Program Manager for Caroline Families First, the CFF Care Coordinator works collaboratively with families and community providers to support and develop a Coordinated Care Plan that reflects the goals and needs of families who are struggling with mental health concerns. The Care Coordinator is responsible to deliver high fidelity wraparound services and is accountable for ensuring the development of the plan and the coordination of services and supports involved with the families.

The Care Coordinator will be responsible for fostering partnerships and relationships with community partners and coordinating and brokering community services as it relates to individualized family care plans.

Please note that this is a **full-time contract** position, working** 35 hours weekly**, with a planned **start date of February 6th, 2023 and an end date of September 11th, 2024.**

**RESPONSIBILITIES**
- As part of a family centred multidisciplinary team, develop, implement, and monitor family care plans under the framework of high-fidelity wraparound that ensures appropriate treatment planning and interventions for children, youth, and their families.
- Serve as the central point of contact for all care team members.
- Demonstrate commitment to the principles of family centred care, family engagement, and family support.
- Develop and coordinate individual family wraparound teams inclusive of formal, informal, and natural supports.
- Provide coordination of care and facilitate communication among the family care teams and with family doctors, allied health professionals, and pediatricians.
- Develop a network of community resources and partners that can provide support and services to children, youth, and their families.
- Effectively assess, manage, design, and support a crisis/safety plan to ensure safety within home/school/community settings.
- Facilitate and lead wraparound meetings by:

- Demonstrating an ability to address and remove barriers.
- Ensuring family voice and choice is heard and respected.
- Acting in an administrative role by distributing care plans and meeting notes.
- Develop and maintain strong working relationships with service providers and agencies involved in the client’s care, ensuring planned actions are adequately followed through as committed.
- Comply with the agency’s reporting and recording requirements and ensure all documentation identified as the responsibility of the CSP is completed in a timely manner.
- Ensure that clinical records and statistics are completed in accordance with the agency’s standards.
- Responsible for maintaining current knowledge of the centre’s policies and procedures, as well as relevant legislation.
- Operationalize client safety through adhering to established policies and guidelines that ensure consistency in the delivery of quality and safe care, and by identifying and reporting any client safety incidents.
- Ensure compliance with the specific requirements of Section 28 of the Occupational Health and Safety Act, as well as any further health and safety responsibilities outlined in the agency’s policy and procedure documents.
- Other duties as may be reasonably assigned.

**QUALIFICATIONS**

**_ Education_**:

- A degree in Social Services or other related discipline from a recognized university, or a college diploma in a related field.

**_ Experience_**:

- A minimum of three years’ experience working in children’s mental health, social services, or primary health care settings.
- Experience in meeting facilitation.
- Good working relationships already established with Halton community service providers.

**_ Skills_**:

- Highly collaborative leadership style and the ability to work in a conciliatory, respectful manner within and between systems that serve children, youth, and their families.
- Strong communication skills, both verbal and written.
- Excellent problem solving and conflict resolution skills.
- Exceptional organizational and time management skills.
- Proficient in the use of technology as required of the position, including working in a virtual environment with MAC OS operating systems experiences preferred.
