Project Coordinator, Age Well at Home

**Position Description**:
The Age Well at Home project has a focus on improving access to home support services and increasing a sense of connection and inclusion in Waterloo, helping low-income and newcomer older adults to successfully age in place. This temporary full-time position will report to the Manager of Community Programming and will lead the development and implementation of the project. This position will work collaboratively with government agencies, internal partners, external/community partners and networks, and directly oversee students and volunteers to ensure project outcomes are met.
**Accountabilities**:
- Provide oversight, coordination and direction related to project deliverables
- Complete a needs assessment to identify priority neighbourhoods in Waterloo, using geographic and demographic data and other factors, where a high concentration of older adults reside (i.e. those most suited for a Naturally Occurring Retirement Community program)
- Conduct market research to inform best practices related to programs and services for low-income and newcomer older adults
- Work collaboratively with the Home Support Services team to support and enhance service delivery for low-income and newcomer older adults
- Alongside the Home Support Services team, develop program eligibility requirements, procedures and support process mapping related to service provision; (i.e. service referral pathways)
- Work collaboratively with internal partners to develop new volunteer positions that support project outcomes
- Lead recruitment, training and onboarding for co-op students and volunteers; Provide ongoing supervision, oversight and direction to students and volunteers supporting project outcomes
- Develop training materials and standard operating procedures for volunteers and students as needed; facilitate training as required throughout the duration of the project
- Facilitate community engagement opportunities in the form of focus groups, workshops, information series, and interviews throughout the duration of the project
- Ongoing consultation with community agencies and social/health networks to support project outcomes
- Develop partnership agreements with external agencies within priority neighbourhoods to facilitate connection to community services (i.e. an office space for volunteers to reside)
- Monitor and report on project budget variances on a regular basis to ensure budget compliance; procurement of equipment and services to support project outcomes and delivery
- Utilize interpretation services, facilitate translation of program and service materials in different languages
- Design and facilitate a series of community information workshops for newcomers
- Assist in the development of a marketing strategy for the project; work collaboratively with Communications and Marketing to facilitate the dissemination of materials into the community to ensure a broad and diverse reach to older adults
- Work collaboratively with defined research partner to develop a common measurement framework; ensure tracking and measurements are in place to demonstrate project success with the use of pre/post surveys, focus groups, interviews, etc.
- Ongoing and active participation as part of a broader community of practice
- Work with consultants to lead the development of a sustainability plan for services beyond project end date
- Monitor and ensure project outcomes are being delivered in a safe and healthy environment, that the City of Waterloo policies and procedures are followed and that the project deliverables are in compliance with accessibility and safety legislation (Occupational Health and Safety Act), Privacy legislation, Connecting Care Act as well as all relevant safe work practices
- Ongoing assessment of community needs; research and analyze trends and best practices to ensure project outcomes are met
- Travel to worksites and community or meeting locations as necessary, using reliable, safe and efficient methods of transporation is required to perform the job
- Other duties as assigned
**Minimum Qualifications**:
- Post-Secondary diploma in a health related field such as Gerontology, Social Work, Health Sciences, Public Health, Occupational Therapy or related field
- Minimum 2 years’ experience in community program development, implementation and evaluation. Equivalent combinations of education and experience will be considered
- Additional certification or education in project and/or volunteer management would be considered an asset
- Project planning, management, and implementation experience required, particularly as it relates to the social determinants of health
- Strong leadership, clear and effective communication (written and oral), team building, organizational, conceptual and interpersonal skills
- Previous experience with recruitment, training, coaching and scheduling students and volunteers
- Knowledge of community support services within the Waterloo area is considered an asset
