Position: Indigenous Patient Navigator
Location: Royal Victoria Regional Health Centre
Supervisor: Director of Health Services or designate
Term: Permanent Full Time

Under the direct Supervision of the Director of Health Services and or designate, the Indigenous Patient Navigator (IPN) will provide support and advocacy for First Nation, Métis, and Inuit patients (in-patient, out-patient care), caregivers and families in the York region, North York, and South Simcoe County region.
**JOB REQUIREMENTS**
All employees of BANAC are required to submit a clean **Criminal Record Check with Vulnerable Sector** upon hire and confirm compliance every year thereafter. All employees are to comply with the Occupational Health and Safety Act (OHSA), its regulations and all occupational health and safety policies and procedures. Due to the requirements of this position, a **Valid Ontario Driver’s License** and use of own insured vehicle is required. Valid **CPR and AED Certificates** are required
**RESPONSIBILITIES**
- Communicate effectively both written and verbally acting as the liaison when engaging with patients, their families and health care teams
- Support patients through triage and the admissions process as needed
- Establish therapeutic relationships, assertive outreach as needed and complete referrals for patients
- Advocate for patients and their families when barriers arise to ensure quality and timeliness of treatment, care of services
- Assist in the facilitation and, the mobilization of outside resources through the transitioning period from hospital care to family and community settings ensuring the right community, professional and cultural supports and services are in place to maximize the wellness and health of the patient
- Assist patients to better understand hospital services, establish communication protocols, and help patients avoid future use of emergency room (ER) where possible
- Assist connecting families to Elders and healers for emotional, spiritual, physical, mental support
- Develop and implement an ongoing communication campaign promoting the Indigenous Patient Navigator service and benefits throughout the health care setting
**KNOWLEDGE/EXPERIENCE**
- Knowledge of medicines, traditional healing, language, ceremonies, and cultural teachings
- Extensive knowledge of health care services with the ability to navigate to make changes
- Experience and knowledge of hospital settings
- Knowledge and understanding of Indigenous culture, values, and history as it relates to health care disparities in Indigenous populations
- Demonstrated ability to work effectively in a multi-disciplinary team environment
**QUALIFICATIONS**
- College diploma or University degree in Health or Social Sciences compiled with 3 years’ work experience in the health and/or Social Work field OR a combination of education, training and/or lived experience as deemed acceptable by BANAC
- Previous experience working within the Indigenous community, combined with knowledge of Indigenous cultural safety and Indigenous cultural practices.
- Registration within a regulatory body (i.e., OCSWSSW) is an asset
- Medical terminology certificate is an asset
- Ability to speak Indigenous language(s) an asset
**SKILLS**
- Excellent communication skills (verbal & written) along with the ability to prioritize and multi-task
- Excellent customer service skills, including strong, respectful interpersonal skills and flexibility.
**PHYSICAL DEMANDS AND WORKING CONDITIONS**
- Work is typically performed in hospital, office and community settings.
- Regular travel to meetings and communities, possible.
- Regular overtime work may be required from time to time.
- Low to moderate exposure to noise in the work environment.
- Regular exposure to a moderate level of tension and anxiety due to deadlines, competing political pressures, and the competing needs and demands for limited resources.
- Periodic exposure to smoke may occur from the burning of sacred medicines: Tobacco, sweet grass, sage or cedar.
