Rehabilitation Navigator

The **Rehabilitation Navigator** provides active leadership to facilitate and coordinate inpatient rehabilitation. Using a collaborative approach and advanced communication skills, the Rehabilitation Navigator builds strong relationships with physicians, Interprofessional teams, patients, families, care team members and system partners.

The Rehabilitation Navigator is responsible for the coordination of patient access to the rehabilitation program. Through thorough assessments of referrals and appropriateness of referrals to the most appropriate rehabilitation stream. The Rehabilitation Navigator is responsible for completing Functional Independence Measure (FIM) admission and discharge assessments, and submissions and edits through the National Rehabilitation Services (NRS) database to the Canadian Institute for Health Information (CIHI). The Rehabilitation Navigator can interpret reports and data to communicate back to the team leading to the improvement of processes supporting access, quality, and safety.

Specific processes within Rehabilitation Navigator scope can include the following:

- Facilitate rehabilitation patient flow within the rehabilitation beds.
- Responsible for completing admission, discharge and edits to the NRS, and submission to CIHI.
- Attend rehabilitation rounds and daily hospital bed meeting to determine and anticipate rehabilitation needs
- Facilitate data collection, and quality assurance related to NRS and rehabilitation quality outcomes.
- Participate in quality improvement initiatives related to rehabilitation.
- Facilitate Post Discharge calls for medicine program.

To support process and system level improvement, the Rehabilitation Navigator contributes to and implements policies and procedures related to the care delivery process, assists in the development of quality and process improvement initiatives, participates in, and shares responsibility for, ensuring that rehabilitation performance is optimized.

QUALIFICATIONS:

- Registered Practical Nurse with current Certificate of Competence from the College of Nurses of Ontario or Rehabilitation assistant.
- Understanding and familiarity with rehabilitation and supporting policies and procedures, and have comprehensive knowledge of rehabilitation.
- Minimum of five (5) years of experience working in a rehabilitation setting.
- Outstanding communication skills and the proven ability to communicate professionally with stakeholders, balancing competing priorities.
- Demonstrated ability to provide support and leadership to advance evidence based inter-professional patient centered care.
- Demonstrated ability to deal with ambiguity in a rapidly changing environment.
- Strong problem-solving, organizational and time-management skills.
- Demonstrated ability to work as an effective member of a multidisciplinary team.
- Participation in continuous quality improvements (CQI) processes (i.e., committees and team meetings) to identify opportunities for improvement and support other quality improvement plans.
- Maintain quality documentation pertaining to submission of all rehabilitation data.
- Mandatory NRS/CIHI training sessions and maintenance of competency and skills as per regulations
- Promote and participate in research activities and evidence-based practices related to NRS
- Provide education and act as a support to staff related to FIM assessments, NRS data, and CIHI reporting
