**Job Description**:
The **Indigenous Coordinator** position aims to build capacity within Indigenous communities through an empowerment and partnership model to address the unique chronic disease care needs of First Nation, Métis and Inuit People. The Indigenous Coordinator will directly support the work of Royal Victoria Regional Health Center (RVH) in the successful development of on-going prevention, screening and patient support for Indigenous community members regarding chronic diseases.

The Indigenous Coordinator will be responsible for managing the development, implementation and evaluation of initiatives relevant to improving community health care including but not limited to diabetes, dementia, heart disease and obesity for First Nation, Métis and Inuit People in the North Simcoe Muskoka Region in consultation with the Barrie and Area Native Advisory Circle (BANAC) as stated in our Letter of Relationship.

**Collaboration, Partnerships & Knowledge Exchange**
- Establish and maintain relationships with FNIM and non-FNIM regional partners to meet the cultural needs of FNIM patients and families, formalizing processes to strengthen culturally safe transitions across the care sector; acute care to community and long term care.
- Collaborate with the Indigenous Patient Navigator and departmental leads to support the implementation and development of Indigenous chronic disease support systems at RVH.
- Collaborate with RVH staff, regional leads, provincial leads, and federal leads as needed.
- Support communication regularly with regional partners, including BANAC and the Indigenous Health Circle on FNIM regional and provincial initiatives.
- Facilitate access to care services and resources for FNIM patients and families.
- Provides knowledge and support to patients to ensure a seamless and culturally safe experience within the hospital particularly with end of life transitioning and spiritual support.

**Project Planning & Leadership**
- Support in the development, design, coordination and evaluation on culture-based prevention strategies to reduce chronic disease risk factors in accordance with Ontario Health’s new First Nation, Inuit, Métis and Urban Indigenous Health Framework.
- Actively participate in regional FNIM specific meetings and events as necessary to facilitate program growth and operationalize key initiatives that are related to meeting patient needs.
- Provide recommendations to management for program development based on patient needs.
- Coordinate community based screening activities using prevention and screening approaches within the Indigenous frameworks, teachings and ways of knowing to allow culturally relevant initiatives to evolve while also blending in Westernized mainstream approaches.
- Support traditional cultural practices in health and healing, facilitating access to cultural, spiritual and linguistic resources.
- Focus on culture based palliative care and end of life strategies that incorporate traditional ways of knowing.

**Communication & Facilitation**
- Communicate with a variety of stakeholders from across the continuum including but not limited to BANAC, Mamaway Wiidokdaadwin and the Indigenous Health Circle.

**Education**:

- Minimum 3 year undergraduate degree or equivalent in a health science, social science, Indigenous studies field or recognized equivalent.

**Experience**:

- Must have experience working with First Nation, Inuit and Métis (FNIM) peoples.
- Experience working in partnership with FNIM communities and urban Indigenous organizations.
- Experience in addressing Indigenous specific health care issues specifically related to chronic disease and end of life care.
- Experience with collaboration and relationship building with mainstream internal and external stakeholders.
- Understanding of the effects of intergenerational trauma and trauma informed approaches to care, and best practice in relationship building.
- Capacity to write funding proposals, create work plans and establish community based advisory groups.
- Expertise to create and facilitate effective presentations and education initiatives to the public and health care professionals regarding chronic disease deliverables including palliative care.

**Competencies**:

- Understanding of First Nation, Inuit and Métis history, culture, health determinants and perspectives on health and well-being.
- Strong insight into the importance of Reconciliation with Indigenous peoples.
- Working knowledge of local FNIM governance systems, protocols, organizations, communities and territories.
- Demonstrated ability to integrate culture into project deliverables and adapt Western perspectives of health and education to align with Indigenous understandings of health and wellness.
- Knowledge of traditional medicines, teachings, language, ceremonies and cultural teachings and how it relates to one’s cancer journey.
