**Student Transition Coordinator**

The Student Transition Coordinator provides support for existing and incoming students considering transfer or pathway activity across a variety of University and College systems. This role offers a bridge between the two institutions, to reduce service fatigue for students, and provide the resources needed to build pathways to success while the student focuses on their academic priorities and goals.

The incumbent provides mentorship on supports and services available at Mohawk College in the Accessible Learning Services offices. This includes access to accommodation supports to foster a smooth transfer at a time that can be very stressful for students.

This role partners with the Accessibility team members to act as a liaison within the accessibility office at both Mohawk College and the other institution to determine student accommodation needs. The role supports multilateral pathways and meaningfully engages the student through the transfer and articulation processes.

The Student Transition Coordinator processes confidential student documentation. The incumbent must have a full understanding of FIPA and PHIPPA legislation, OHRC policy on the duty to accommodate persons with disabilities, along with a full understanding of the student record keeping system used in Accessible Learning Services

**RESPONSIBILITIES**:
The duties of this position will include, but are not limited to the following:

- Transition Coordination_
- _ _Develop strategy and build content for program design
- Develop partnerships with other post-secondary accessibility and counselling offices across Ontario to become a central contact with other accessibility offices across the Ontario Post-Secondary system
- Assist students with disabilities and mental health diagnoses in better understanding their skills & strengths and connect them to opportunities which will allow them to explore additional education pathways both virtually and in person
- Provide mentoring, resources, and help to guide students through all pathways from college to university, college to college and vice versa
- Partners with Mohawk College ALS team to support completion of intake forms, gather documentation, and arrange appointments
- Meeting with students to offer check in appointments using structured interview and referral process
- Reception and Clerical Duties_
- _ _Engages in internal and external promotion of departmental services and programs
- Processes requests for information/documentation from student files and prepares letters in draft for Manager’s signature to release contents of student files for students transferring to other post secondary institutions
- Collects data on:

- The number of credit transfer students for each academic year
- The number of virtual and in person appointments
- Feedback from accessibility at Mohawk and other institutions (receiving or sending) on satisfaction of support provided by role
- Student satisfaction with support services via short surveys
- Facilitates ongoing and timely access to non-urgent students to the ALS team.
- Facilitates the placement of information on the electronic record management system and departmental common drives.

**QUALIFICATIONS**:
The successful applicant must have:

- 3-year Diploma/Degree in a related field of studies
- Safe Talk, Mental Health First Aid, Non-Violent Crisis Prevention and Intervention courses
- Experience working with persons with disabilities including mental health diagnosis to facilitate their academic social and career development which includes training in AODA, OHRC Policy.
- Experience providing client services programs, advocacy, coaching, and problem solving.
- Experience with current computer software (e.g., MS office) and data management systems, including generating reports, performing data queries, maintaining databases (e.g., spreadsheets).
- A minimum of three years of practical experience, in a service-oriented environment within an educational setting
