To help fulfill its mission, LOFT is hiring a **Coordinator - **Positive Service Coordination**.**

**JOB SUMMARY**:
To Coordinate the overall service delivery and day-to-day operations of the housing and support programs at a specific site. To provide a range of assistance and support in facilitating the transition from being homeless to having stable housing for those who identify as HIV+ or Transgender, with mental health &/or substance misuse.

**WHAT YOU WILL DO**:

- Assessing the individuals referred to the program as to their appropriateness and then determining their daily living requirements or needs; negotiating a plan of support and recovery which includes identifying the priorities, goals and required supports for their transition from homelessness to stable housing, in conjunction with other professionals and staff; re-evaluating the level of support provided and interviewing members as to their suitability for alternate and partner supports; participating on case conferences and providing case management services to those who require these services; liaising with other service providers as needed; maintaining and enhancing current partnerships.
- Promoting the personal empowerment of each resident by building a rapport through coaching, mentoring and support, enabling each to meet the daily requirements of their living environment; directly assisting with their daily living activities, (e.g. Hygiene, shopping, cooking, medication management, cleaning, budgeting); establishing and maintaining a support network of social, family, medical and psychiatric community links and resources; advocating on their behalf in navigating the social services and health care systems to ensure accessibility and availability of services; providing 24 hour crisis support on a rotational basis through a pager or similar system; mediating issues or conflicts between residents, or residents and neighbours, teaching conflict resolution; following-up on issues with residents to ensure resolution;
- Providing direction and support to staff involved in direct service delivery; scheduling staff, assigning duties, assisting in the selection, orientation, training and performance management of staff; assisting with recruiting and hiring; supervision of staff; developing and implementing operating procedures for the site and ensuring compliance with established standards; maintaining a general awareness of the daily activities, identifying staffing issues;
- Monitoring the operations of the site and ensuring compliance with property standards and fire codes; co-ordinating the acquisition, use and maintenance of furniture, supplies and equipment, contacting appropriate source for repairs; maintaining accurate records and files on the provision of service to residents/clients, ensuring that incident reports have been completed and processed; maintaining the petty cash;
- Participating in the annual program planning process which includes ensuring regular partnership meetings and maintaining the MOU, operating plan, client satisfaction feedback process, and other research activities; identifying gaps in service and recommending solutions, participating in meetings, planning and evaluation initiatives, committees, events and activities; compiling program evaluation reports and statistical information, inputting as required;
- Organising and participating in meetings, resident association activities or other group activities which promote the wellbeing of the residents and the smooth operation of the houses; conducting and facilitating group meetings; providing other related assistance as required.

**WHAT YOU WILL DO**:

- Knowledge is required of the issues faced by adults with special needs and of a marginalised client group which can include addictions, cognitive impairments, HIV/AIDS and mental health-specifically Trauma.
- Experience in program development, planning and evaluation. An understanding is also required of the therapeutic relationship with an ability to negotiate with clients on goals and priorities.
- Excellent interpersonal, communication and counselling skills are required to provide team leadership and support and to engage the clients in some highly personal areas.
