The Intake & Family Support Coordinator responds to incoming calls from families of children with a diagnosis of Autism or Autism Spectrum Disorder (ASD), for entry into OAP (Ontario Autism Program) and Lumenus Core Clinical Services.

The Intake & Family Support Coordinator ensures adherence to eligibility criteria and supports caregivers through the intake process. Services are offered to families in in-person and virtual formats.

KEY RESPONSIBILITIES:

- Provides intake services for children and youth with Autism into the Ontario Autism Program (OAP) and Lumenus Core Clinical Services
- Respond to incoming calls from families of children and youth with Autism Spectrum Disorder
- Ensure adherence to eligibility criteria
- Supports caregivers through the intake process
- Conducts intake assessments and creates service plans
- Provides telephone support and referral to other services when needed
- Supports families with clients in the Ontario Autism Program and Lumenus Core Clinical Services
- Ensure the flow of information between Lumenus and other community agencies as required
- Provide supports for parents; including the provision of written materials and assisting in access to community support
- Provide case coordination services and interim support to families as needed
- Assist in the development of parent resources, such as handout, tip sheets, etc.
- Prepare and present parent workshops and training
- Support the families’ transition to other assessment/treatment services as required
- Liaise with caregivers on a regular basis
- Maintains accurate client information in electronic databases
- Participates in the analysis of data and compiling of reports as required

**QUALIFICATIONS/EXPERIENCE/KNOWLEDGE**:

- Bachelor’s degree in psychology, social work, or related field;
- Four years’ experience working with children/youth with developmental disabilities or diagnosed with Autism Spectrum Disorder;
- Knowledge of Autism Spectrum Disorder;
- Knowledge of Evidence Based Parenting/Caregiver mediated programs such as Triple P, Project ImPACT is an asset;
- Knowledge of typical, and atypical child and adolescent development;
- Knowledge of Applied Behaviour Analysis (ABA) principles preferred;
- Strong interpersonal skills and ability to consult to families and community partners;
- Excellent communications skills, both written and oral;
- Time management, planning, and organizational abilities;
- Good clinical judgment and problem-solving skills;
- Ability to work independently as well as with multi-disciplinary teams both within the agency and the community;
- Cultural competency and good observational skills;
- Flexibility in service delivery;
- Understanding of child welfare issues and policies;
- Use of Microsoft Office (MS Word, PowerPoint, Excel, and Outlook);
- Demonstrated interviewing and active listening skills;
- Skill in using Client Information Systems (i.e. CIS/CYSIS).

**ADDITIONAL REQUIREMENTS**:

- Ability to work evenings and Saturdays (2 Saturdays per month on-site) as required by program and client need
- Flexibility to work between program sites
- Satisfactory clearance under the Vulnerable Sector Screening program
