**Role Overview**

**About The Assertive Housing First Team (AHFT)**

The AHFT utilizes an interdisciplinary team approach to provide Housing First support to individuals who are homeless and living with mental health challenges. A portion of the team's service capacity is dedicated to individuals referred to AHFT through mental health court support services or one of three area correctional facilities (Toronto South Detention Centre, Toronto East Detention Centre, or the Vanier Correctional Centre for Women).

AHFT provides support with rent, housing and intensive wrap-around support helping individuals live safely and successfully in the community of their choice, achieve improved personal health and wellness, and reduce pressure on the justice system.

**Summary of Duties**:
Responsibilities of this position include but are not limited to:

- Provide recovery oriented support through lived experience of mental health and/or addiction challenges
- Work collaboratively with individuals and the team to develop recovery and goal plans
- Teach, coach and provide outreach to individuals
- Support individuals to live independently in the community as they consider and determine their own unique goals and objectives
- Provide recovery-oriented and/or harm reduction based services in a manner consistent with Cota's Mission, Vision and Values
- Facilitate various groups: social recreational, educational and/or life skills groups

**Summary of Qualifications**:

- Consistent employment OR volunteer work OR training in the past 6-12 months
- A minimum of High School Diploma
- Lived mental health, addictions experience and/or justice involvement
- Experience providing peer support to people with lived experience (asset)
- Completion of Peer Support training such as WRAP or Pathways to Recovery (asset)
- Experience facilitating groups (asset)
- Excellent listening and communication skills
- Ability to be flexible and make recommendations to resolve issues
- Ability to effectively manage personal mental health and/or addictions issues
- Demonstrate self-awareness of recovery journey and purposeful self-disclosure
- Knowledge of Psychosocial Rehabilitation principles, especially empowerment
- Computer efficient (MS Word, E-mail, Internet

**Other Requirements and Working Conditions**:

- Conduct visits in the service recipient's homes, hostels, hospitals and other community settings, sometimes working in difficult conditions, including hoarding and pest infestations
- Comfortable working with the broad range of people we support groups
- Working hours Monday - Friday 8:30am - 4:30pm
