We are looking for a Program Lead who collaborates with Antimicrobial Stewardship and Infectious Disease Physicians, prescribers, nursing as well as Pharmacy Services departments across SH and UHN to develop the strategic approach for the ASP team to deliver various initiatives, including: audit and feedback with prescribers and pharmacists, inter
- and intra
- professional education, evaluation of compliance with guideline/best practice regarding appropriate use of antimicrobial agents (in their role as a pharmacist with expertise in antimicrobial use).

The joint Sinai Health-University Health Network Antimicrobial Stewardship Program (SH-UHN ASP) blends clinical infectious diseases, microbiology, epidemiology, and pharmacy with quality improvement, policy, organizational behaviour, implementation science, behavioural economics, education, medical informatics, and data science to improve antimicrobial therapy.

**DUTIES AND RESPONSIBILITIES**:
The Program Lead mentors and supervises the Sinai Health ASP pharmacotherapy specialist, including providing direction and ensuring alignment of work product and activities with the program’s goals and objectives. The Program Lead is responsible for program budget planning and, on a day to day basis, oversees a team to use antimicrobial stewardship principles and methods to assess for risk and to develop, implement and evaluate strategies, where if not implemented, may result in the occurrence of critical incidents or poor clinical practice. This role is essential to building our capacity to guide the appropriate use of antimicrobials.

**Clinical Practice, Education and Teaching**
- Leads Antimicrobial Stewardship patient care rounds and provides patient specific feedback to the clinical team (audit and feedback)
- Participates in the development, implementation and evaluation of evidence based empiric treatment algorithm guidelines
- In coordination with the Pharmacy Department ensures ongoing antimicrobial formulary review and establishing an ongoing antimicrobial usage benchmarking program
- Leads knowledge and capacity building across the organization on the value of antimicrobial stewardship in patient safety/quality improvement
- Liaises with the Infectious Disease service and provides pharmacotherapy and stewardship teaching to the Infectious Disease trainees, pharmacy learners and learners in all health professions
- Participates in the Tri-hospital Infectious Disease Service patient care rounds
- Participates in conferences and regional planning groups and represents the hospital and the University at provincial, national and international meetings

**ASP Program Development, Quality Improvement and Innovation**
- In collaboration with the Senior Director and Medical Director, translates Sinai Health’s strategic directions and operational priorities into program-specific goals and objectives
- With program leadership, establishes quality improvement initiatives to improve antimicrobial use, lower antimicrobial resistance, increase patient safety and promote cost savings
- Identifies and collaborates on quality improvement and research
- Mentors pharmacy students, residents and fellows research projects

**ASP Program Management**
- In collaboration with the Medical Director, works with clinical leadership and departments requesting ASP consultation and support to manage prioritization of resources
- Develops and implements strategies for team growth and organizational impact
- In collaboration with the Medical Director, oversees and mentors SH ASP pharmacist(s)
- Co-leads SH-UHN ASP Advisory Committee with the Medical Director and UHN ASP Program Lead and serves as a member on a number of hospital committees related to patient safety, for example, the Infection Prevention and Control Committee, Clinical Operations and Support Function forum, Antimicrobial Subcommittee.
- Manages program budget planning and all human resource related matters including resourcing, recruitment and performance reviews
- Job Requirements- Bachelor’s degree in Pharmacy (BScPhm) or Doctor of Pharmacy (PharmD)
- Certificate of competency from the Pharmacy Examination Board of Canada
- Active registration with the Ontario College of Pharmacists (OCP) Part A
- A minimum of 5 years of experience working in a hospital pharmacy
- Proven experience achieving ongoing quality and process improvement strategies
