The Albany Medical Clinic has been operating for 75 years and continues to provide comprehensive primary care services to our community. Our clinic of 35 family doctors and more than 40 specialists offers diagnostic imaging, walk-in clinic, lab, and pharmacy all under one roof. Our team serves over 50,000 patients from the Broadview-Danforth and Mid -Toronto community, as well as the larger East Toronto community.

We have an opening for 10 month contract role as a Quality Improvement Project Coordinator with the possibility of an extension. The estimated start date is May 1, 2023.

**ABOUT THE ROLE**:
Albany Medical Clinic is looking for a Project Coordinator who will be responsible with providing support with the implementation of a quality improvement project to optimize asthma care in the clinic. The coordinator will engage in patient outreach and support, will work directly with clinic staff, and will analyze and summarize data related to the project. There will also be extensive interaction with the University of Toronto-based medical research team that is helping to coordinate the project.

**RESPONSIBILITIES**:

- Collaborate with clinic leadership, physicians, clinic staff and external stakeholders to develop and implement strategies to ensure the success of the project
- Produce and share implementation strategies (with direction from clinical team), knowledge translation resources, and reports with key performance indicators (KPIs) with stakeholders
- Communicate updates regarding the project to internal and external stakeholders
- Greet patients and answer inquiries in a professional and respectful manner to boost patient engagement
- Maintain patient confidentiality when working with personal health information (PHI) and respect professional boundaries with clinic staff and patients
- Conduct thorough interpretation and analysis of all data collected for the project
- Assist in creating and optimizing patient and provider questionnaires for measurement of quality indicators
- Work efficiently on PS Suite EMR and Ocean for patient outreach (training will be provided)
- Engage in any other related duties to optimize efficiency at the clinic

**Working Conditions**
- Normal office environment. Standard work week of 35 hours onsite with possibility of some remote work.

**QUALIFICATIONS, EDUCATION AND REQUIREMENTS**:

- Minimum Education: Completion of undergraduate studies or in the process of completing undergraduate studies
- Excellent communication and interpersonal skills to build and maintain professional relationships.
- Strong problem-solving and troubleshooting skills
- Effective organization, time-management and attention-to-detail skills
- Ability to work in a fast-paced environment and manage priorities accordingly
- Uphold strong values of honesty and integrity
- Comfortable with using Microsoft Office (Excel) to collect, interpret, and analyze data
- Ability to commute to Toronto, ON
- Experience working with personal health information and ability to work with clinical patient information systems (e.g. PS Suite EMR, Ocean) is an asset
