Overview:
We are seeking a dedicated and compassionate Case Manager/ Facilitator to join our team. As a Case Manager, you will be responsible for delivering and managing the care and support of program participants. Our newest program RYSE (Reaching Yezidis: Empowerment and Support) will provide Yezidi women survivors of genocide and sexual slavery access to culturally specific, trauma informed Life Skills training, community resources, one-to-one case management sessions encompassing informal counselling, goal/action planning, recommended interventions and transitional supports.

The Case Manager/ Facilitator delivers a hybrid of in-person and remote training and support services that amplifies the impact of trauma-informed workshops, peer-to
- peer groups and individual case management to inspire personal development and create positive change. Reporting to the Programs Director, the Case Manager/ Facilitator will be responsible for leading a group of 15 women 3 days a week, through a series of culturally sensitive activities, life skill training based on restorative practices and discussions guided through a trauma informed, anti-oppressive lens.

Schedule: Contract (28 hours/week) - Hours/ days may vary depending on the program and client needs. This is a contract position until December 31st with a possibility for extension.

A Dynamic Facilitator - you bring a range of knowledge and experience delivering gender based violence and life skill content including customization and facilitation of support groups

Proven Case Manager - you have a broad knowledge of the impacts of trauma and gender-based violence and experience with refugees, newcomers/immigrant women’s issues and have thorough knowledge of community resources and partner agencies in York Region and the GTA.

Independent - you are self-motivated, demonstrate excellent time-management abilities, problem solving and decision-making skills. You enjoy working collaboratively with team members, work with interpreters (to facilitate accessibility to group participants) wherever necessary and are able to organize and reorganize your day to meet conflicting priorities and deadlines.

Duties/Responsibilities/Procedures
- Develop, update and deliver workshops to maximize program effectiveness and ability to meet a diverse range of participant needs
- Engage, motivate, and maintain rapport with all program participants and be flexible in order to accommodate barriers or vulnerabilities
- Assist program participants in achieving their immediate and long-term goals, provide outside referrals to meet their needs and offer wraparound supports through community service providers
- Remain up to date with respect to evidence based programming and current trends in the social service
- Frequent and open communication with Director and RYSE team for the purpose of program delivery, reporting, maintaining files, processes etc.
- Adhere to agency policies and procedures, Funder guidelines and standards for program delivery
- Maintain all documentation, statistical data entry and reporting requirements, including funder reports and outcome measures as required
- Attend advisory committees, meetings, workshops, seminars and conferences that relate to the work of the Case Manager/Program facilitator position
- Attend and contribute to staff events, meetings, and agency initiatives
- Other jobs as assigned by the Executive Director or Director of Programs

Job Specifications:

- Provide and maintain a clear Criminal Record Check and Vulnerable Sector Check
- Minimum 2-3 years of experience facilitating and leading groups
- College diploma and/or University degree or equivalent in Social Services, Psychology, education fields that intersect with the work done at Project Abraham
- Valid Ontario Driver’s License (G-class) and access to a vehicle
- Experience in post-traumatic stress counselling and crisis intervention is a strong asset

**Job Type**: Part-time

**Salary**: $35.00 per hour
