**Responsibilities**
- Review referrals from probation and parole to determine eligibility of clients for the MAPS program
- Meet with clients to set out goals and an individual learning plan to meet the needs identified by the client, probation/parole and the Ontario Common Assessment of Needs (OCAN) assessment
- Develop and maintain MAPS program guidelines
- Collect data to ensure program outcomes are tracked and meet funding requirements
- Recruit, train and coach the peer supporters in their role to support individuals attending the MAPS program
- Schedule, organize, facilitate and assess training for peer supporters
- Participate in focus groups to determine co-designed curriculum
- Complete and maintain accurate peer supporter records and reports
- Provide information and promote the peer support model
- Support peers in decision making processes
- Provide individual or group peer support for individuals attending the MAPS program
- Document client contacts, create daily logs, summaries and complete reports in a timely manner, as outlined in the CMHA Durham policies and procedures
- Participate in meeting accreditation standards
- Perform other tasks as assigned by management

**Requirements**:

- Transferable lived experience
- Post-secondary education (i.e. human and social services) and a minimum of one-year related experience or equivalent combination of education and experience
- Knowledge the Mental Health Act, Health Care Consent Act, Employment Standards Act, social service benefits and related legislation in Ontario
- Ability to work collaboratively with a team to respond to individuals who may require immediate services/supports
- Ability to work sensitively with people of diverse backgrounds who experience poverty/ discrimination
- Ability to inspire and motivate others
- Ability to develop and maintain a positive relationship with individuals
- Ability to adapt to changing circumstances
- Excellent communication (written and verbal) skills
- Ability to organize and present information
- Ability to participate as a member of a high-performance multi-service team
- Works a variable work schedule, which may include evenings and weekends, as required
- Knowledge of MS Office (Word, Excel, PowerPoint, Access and Outlook), as well as the internet
