Nursing Coordinator

We are currently seeking an experienced LPN or RN Coordinator for our Home Medication Management program. This role is integral in ensuring safe, effective medication management for community-dwelling patients transitioning from acute care settings to home care.
**Responsibilities**:
- Assess and monitor patients' medication compliance, communicate with healthcare providers, and identify necessary adjustments or interventions.
- Provide medication administration support, education, and oversight in accordance with established protocols.
- Liaise with acute care teams, community healthcare professionals, and family members to ensure a comprehensive approach to patients’ medication management.
- Facilitate patient referrals to other community services based on patients' needs and best practices
- Oversee and manage the accurate documentation of patient assessments, medication administration, and changes in health status.
- Participate in relevant committees, advocating for home-based care and contributing to strategic planning and decision making.
- Coordinate care provided by nursing staff, ensuring tasks are appropriately assigned and the quality of care is maintained.
- Perform other related duties as assigned by management.
**Requirements**:
- Current and valid registration with the British Columbia College of Nurses and Midwives (BCCNM).
- A minimum of two years of experience in a supervisory or lead role, preferably in a community or home health setting.
- Experience in home medication management, or a similar field.
- Strong communication and interpersonal skills.
- Excellent problem-solving and decision-making abilities.
**Compensation**:
This is a full-time position with a salary range of $70,720 - $93,600 per annum.
