Skilled Nursing Care Coordinator

We are seeking a highly skilled and dedicated Registered Practical Nurse to join our team at Mon Sheong Richmond Hill Long Term Care Centre.
About the RoleThis is a part-time, bargaining unit position that involves planning, delegating, and supervising care given by non-registered nursing personnel within your assigned unit. You will be acting under the direction of the RNs and reporting to the Director of Resident Care or designate.
Main Responsibilities- Provide high-quality nursing care and clinical interventions in the assessment, planning, implementation, and evaluation of resident-driven care;
- Meet the physical, psychological, emotional, spiritual, cultural, and social needs and expectations of residents as assessed and planned by the interdisciplinary team;
- Develop and implement individualized care plans for residents, ensuring their comfort, safety, and dignity;
- Administer medications, control narcotics, apply treatments and dressings, transcribe physician orders, and re-order medications as needed;
- Supervise and support non-regulated staff on your assigned Resident Home Area (RHA), adhering to the standards of the College of Nurses of Ontario;
- Assist with training and development needs of non-regulated staff, monitoring their compliance with Home's policies and procedures;
- Participate in meetings and conferences with staff on your assigned RHA;
- Certificate of competence from the College of Nurses of Ontario;
- Valid CPR and First Aid Certificates;
- Certification in Gerontology and current membership with GNA and RNAO (desirable);
- Minimum one year experience in geriatric, rehabilitation, or long-term care (preferred);
- Adherence to professional competencies, including caring and respectful behavior, clinical knowledge and skills, assessment skills, communication skills, conflict management skills, and organization skills;
- Demonstrated supervisory and team leadership skills;
- Proficiency in oral and written English; ability to speak Cantonese/Mandarin (an asset);
- Willingness to work rotating Day/Evening/Night shifts with alternate weekends;
- Relevant computer literacy;
- A competitive salary range of $80,000 - $110,000 per annum, depending on qualifications and experience.
