**Summary of Job**:
We are seeking a compassionate and organized Client Service Coordinator to join our team at BIAD. Reporting to the Program & Services Manager, the Client Support Coordinator (CSC) is a vital frontline position with BIAD. The CSC role assists individuals living with acquired brain injuries (ABI) in understanding their needs for support, identifying and addressing client goals and developing a care plan. Your primary responsibility will be to coordinate and deliver client-centered support services, ensure individuals are connected to the necessary resources and assistance to clients as they navigate their journey through recovery and community re-engagement.

**Hours**:Full-time. Occasional requirement of availability for special events evenings/weekends.

**Responsibilities, include but are not limited to**:

- **70% - Client Support**_
- Serve as the primary point of contact for clients, providing differing levels of services based on the client’s individual needs and the ABI Services Delivery Framework. Ensuring services align with the appropriate BIAD programming as outlined below.
- **Community Support Services Program**:

- An agile outreach service that functions as an accessibility guide to provide service connection and support in the community. Transitional support services providing clients assistance transitioning from OW to ODSP or CPP-D. Engage the use of technology in client’s homes to aid in memory support and daily life.
- **Homelessness Prevention Program** - Provide services and supports to assist vulnerable or at-risk clients with support in obtaining or retaining housing.
- **Supported Independent Living Program**:

- Provide one to one, goal directed, support based upon a client’s identified level of support requirements.
- Conduct assessments of clients' needs for support and goals, including housing, income, supports outside of the Brain Injury Association, and impact of ABI, to develop individualized support plans. Staff will implement, review, and assess on a regular basis.
- Assist and support clients with skills development in planning, organizing, and completing necessary documentation for services such as: health links, coordinated care planning, ODSP, CPP, OW, supportive and RGI Housing.
- Uses sound judgement while carrying out job duties in a professional manner.
- **20% - Connecting with community service providers, improving understanding and awareness of ABI’s through education, and advocacy**_
- Stay informed about advancements in brain injury research and community resources to ensure the provision of up-to-date and relevant information to clients.
- Coordinate and facilitate access to a range of support services and community resources, based on clients' unique requirements. This may include collaboration with healthcare professionals, social workers, and other service providers to ensure a holistic approach to client support and maximize the client’s independence through effective interventions. Connection to community resources, securing housing, food security programming, and health links as required.

- Participate actively and constructively as a team member in all staff and team meetings, client file audits, case conferences and training sessions as required to ensure continual program development.
- Maintain accurate and confidential client records, progress notes, and statistical data to track and evaluate the impact of services provided.
- Comply with all applicable agency policies and procedures. Specifically, as it relates to the programs as it impacts on the responsibilities of this position including legislation related to privacy and the release of personal information. This will include following appropriate privacy legislation such as PHIPA.
- Perform other duties and responsibilities as assigned.

**Qualifications**:

- Bachelor's degree or diploma in social work, social services, justice services, addictions & mental health or a related field.
- Experience working with individuals and families affected by brain injuries, disabilities, or related conditions. Knowledge of brain injury rehabilitation, community resources, and relevant support services considered an asset.
