Position Overview

CarePartners is expanding its workforce and is in search of Service Coordination Specialists to facilitate outstanding customer service and communication among patients, their families, and our dedicated staff. Our Service Coordination Specialists play a crucial role in ensuring that the scheduling of Personal Support Workers, Nurses, and Therapists aligns with service delivery standards while also managing administrative tasks related to client services efficiently.

We currently have openings for Part-Time positions

Work hours consist of a defined shift schedule, including days, afternoons, and evenings.


Part-Time roles require shifts every other weekend, with opportunities to take on additional shifts.

Responsibilities of the Role

Act as a liaison between patients/caregivers, their families, Personal Support Workers, team supervisors, funders, finance, and other CarePartners personnel Receive routine referral/intake information from funders and private-pay clients Develop and coordinate service schedules while considering relevant factors Notify workers of new assignments and any changes to their schedules Input relevant information into the automated information system/database Generate schedules and reports regularly and upon request from the Supervisor Handle inbound telephone calls and email inquiries related to services Perform various administrative duties as needed Undertake other responsibilities as required

Qualifications

A strong commitment to making a positive difference in the lives of patients Post-secondary education is preferred, along with one (1) year of relevant experience (such as administrative or customer service) Exceptional interpersonal and customer service abilities Capacity to work independently as well as collaboratively within a team Strong proficiency in Microsoft Office, particularly in Word and Excel Keen attention to detail and the ability to learn new systems and databases Effective verbal and written communication skills Availability for non-traditional hours, including evenings and weekends Experience in the healthcare sector is an asset A clear background check is required
