Quality Improvement Coordinator

Sinai Health is seeking a Quality Improvement Coordinator - Patient Reported Outcome Measures (PROMS) to coordinate the design, development, and evaluation of a data driven approach towards improving patient experience and satisfaction. The role will advance Sinai Health’s use of patient centred measures including patient experience (PREMs), patient satisfaction, compassionate care and patient reported outcome measures (PROMs). Measurement and research are critical to drive changes in behaviours and processes and to build evidence related to patient care, engagement practices and outcomes. Measurement can enable informed decision making for patients and family caregivers.
**Duties & Responsibilities**:
Implement Measures of Patient Level Experiences, Goals and Outcomes
- Develop and implement condition specific and generic, patient-reported outcome measures
- Develop methods that collect more detailed data about patient experiences and outcomes, such as video to capture interactions and observations
- Implement measures that assess patient and family engagement in direct care and patient and family partnership at the organizational level
- Analyzes and interprets data to assess organizational and unit level opportunities for improvement
Create feedback mechanisms (using measures) to help plan patient care, provide real-time, personalized feedback to clinicians and organizations to drive change
- Integrate existing and new measures of patient-reported outcomes into clinical practice so that clinicians can better understand patient life experiences and quality of life concerns
- Implement solutions to collect point-of-service data about patient experiences and levels of engagement, using tablets or other mobile technologies
- Create patient experience maps charting staff and patient input to understand expectations at various care points and identify opportunities for improvement
- Create dashboards with benchmarks for quality improvement efforts
Participate and lead in research initiatives, in partnership with the Sinai Health Science of Care Institute, on the impact of patient and family engagement on organizational outcomes
- Maintains familiarity with current literature and academic evidence about patient centred measurement
- Advance patient and family experience measurement within healthcare
- Integrate new and existing measures of patient-reported outcomes into clinical practice
- Partner with patients and families in health and healthcare research
Other duties as assigned within the Quality and Patient Experience portfolio.
- Job Requirements**Education**:
- Successful completion of a Master’s Degree in a related field from an accredited educational institution (e.g. epidemiology, public health, health administration, data science, health sciences, business, etc.) Bachelor’s Degree or a combination of equivalent experiences with a strong focus in Patient Experience and Engagement or Community Participation
- People-Centred Care Leadership Certificate, Planetree Certification for Excellence in Person Centered Care, The Beryl Institute Learning Programs, IAP2 Certificate in Public Participation
- Previous participation in QI training opportunities e.g. IDEAS program, IHI Open School, C-QUIPS, Patient Safety Education Program (PSEP) an asset
- Strong project management skills (PMP is an asset)
**Qualifications**:
- Demonstrated experience with the use of patient centred measures
- Knowledge of current issues and practices related to patient centred measures including PREMs and PROMs or patient activation measures
- Expertise in patient and family centred care principles
- 3-5 years of experience in advancing public / community engagement and participation. Within the context of a healthcare environment with patients / clients and Family and asset.
- Experience with and demonstrated knowledge of patient/community engagement methods, quality improvement methodologies (e.g. PDSA, Experience-Based Co-Design Methodology)
- Healthcare experience (at least 2 years) including an understanding of system level thinking and design
- Strong group facilitation skills: knowledge and experience with experience based co-design methodology an asset
- Exceptional verbal and written communication skills to effectively promote key ideas
