**JOB SUMMARY**
- The mandate of Long Term Care Services is to provide resident care based on the needs of individuals, as determined by medical, nursing, functional and psychosocial assessments, and on each resident’s expressed needs. The care provided in each Home includes specialized, restorative, supportive and palliative care for persons with dementia, disabilities and health problems who cannot live independently in their homes, and whose needs cannot be met in the community.- You are responsible for providing expertise and guidance to LTC Services staff in the areas of quality management (improvement, assurance, techniques, tools, and analyses), risk mitigation, regulatory compliance, evidence-based practices, infection prevention and control, credentialing, and personal health information protection for Long Term Care Services. You will lead and monitor a data driven quality framework in LTC for performance management and quality improvement outcomes.- You are responsible for supporting the accreditation cycle, evaluation of required programs (ensuring commitment to evidence-based and leading professional practices) and quality care plans for Long Term Care services. This includes the responsibility of contributing to and updating a balanced scorecard, data elements, monitoring and managing the continuous quality improvement processes for programs, projects, and activities that specifically target policy development and tools for performance measurement. You will develop strategies to promote continuous quality improvement for care to residents and safety of staff; identify and monitor/prevent potential risks in all care practices through infection and control measures; and support as needed on any Ministry of Long-Term Care compliance issues and high-risk situations while ensuring that legislative and regulatory requirements are met.- You are also responsible for managing the unit’s human and financial resources, and stakeholder and partner relationships as required.
**EDUCATION AND EXPERIENCE**
- Completion of a 4 year university degree in Health Sciences, Business Administration, Public Administration or another related field- Minimum of 5 years of related experience in LTC practices, programs and evaluation, preferably with research and data analysis, including a minimum of 2 years in the management of human and financial resources and direct supervisory experience
- Desirable Qualifications:
- Certification in planning and evaluation
Education and training in the following specialty areas: quality management, risk management, patient safety, infection prevention and control, and/or performance management/program evaluation**KNOWLEDGE**

- Legislation, regulations, policies, standards and guidelines relevant to the work
- Industry trends and developments in Quality Care/LTC practices
- Knowledge of risk management and quality improvement in a health care setting;
- Quality assurance as it relates to legislative compliance and risk management
- Business administration concepts, theories, principles and methodologies
- Corporate policies, guidelines and practices
- The various Collective Agreements in place at the City
- Understanding of the programs/services and operations of other City areas, and the inter-relationships between them and own work area
