**Program: Supportive Housing - Community Supports & Outreach**
The WoodGreen Community Supports and Outreach (CSO) program serves clients 18 years old and above living with mental health challenges and addictions, who requires a more supportive housing environment. The CSO program offers individualized case management, daily community development/engagement, and pest control & hoarding management. These services are offered 5 days a week, 8 hours a day. The clients live in four WoodGreen owned housing which are small-scale apartment buildings in the areas of East York and Downtown Toronto. It also includes clients living in independent units scattered throughout the city that are being assisted with rent supplement by WoodGreen Community Housing.

**What You Will Do**
- Conduct building and resident needs assessments on an ongoing basis to ensure program initiatives are inclusive, relevant, and empowering for resident communities.
- Facilitate programming and workshops in person.
- Supports tenants to identify skills development needs and facilitate opportunities for development or assists individuals to access learning to develop the skills.
- Assist tenants in identifying any issues, problem areas, and relevant needs.
- Facilitate, support, and encourage engagement of tenants in the ongoing management of their building and participation in community and community building initiatives.
- Provide tenants with access to necessary information that may impact their housing, health, community, and general wellbeing.
- Work collaboratively to plan, promote, and implement social recreational groups and/or individual activities and events involving WoodGreen residents and outreach clients.
- Work with tenants to develop community policies, structures, and forums to address issues and needs identified by tenants and community members.
- Travel to different WoodGreen locations occasionally as needed and/or work for extended periods of time at one location, if needed.
- Set up/tear down training room tables, chairs, computers, other equipment, and materials.
- Maintain responsibility for the cleanliness and hygiene of all workshop rooms and equipment.
- Engage with role model, and facilitate positive relationships with residents and community.
- Develop Community Building & Engagement programs to engage residents, to encourage social interaction and inclusivity, and to support empowerment and self-sufficiency.
- Liaise with staff and tenants to coordinate information sessions on tenant rights and relevant topics to community safety and wellbeing.
- Identify and foster opportunities for peer-to-peer learning, skill-sharing, and meaningful community engagement.
- Work with support providers and tenants to develop understanding about the respective roles and responsibilities of each party.
- Carry out administrative procedures and maintain appropriate records with respect to administration and community development.
- Undertake operational expenditures aligned with assigned budget and submit monthly expenditure reports and necessary documentation for expense processing.
- Uphold confidentiality policies regarding tenant information and financial matters.
- Prepare reports and updates as required.
- Ensure the anti-racism objectives and community development principles of the organization are achieved within the program.
- Participate in regular supervisory, staff and unit meetings.
- Carry out additional duties as identified by the Manager, Supportive Housing Program.
- Lift and transport moderately heavy objects, such as computers and peripherals, training materials and supplies.

**What You Bring to the Team**
- Bachelor’s degree in social work or a related undergraduate degree required.
- 2+ of direct experience in social services working with individuals who have a history of homelessness, low income, and mental health challenges.
- Demonstrated knowledge and experience with needs assessments and program planning and implementation.

- Demonstrated cultural competency with ability to communicate and engage with individuals from diverse ethno-cultural backgrounds and levels of abilities.

- Excellent group facilitation skills.
- Excellent oral, written communication, and documentation skills.
