**Job Summary**

As a Community Support Worker, you will be part of a team that co-ordinates a support network and linkages for adults with serious mental health, physical health, and addiction challenges. Staff are expected to be an integral part of the team and offer a variety of functions in support of the social and mental well-being of clients within a safe and supportive atmosphere. A key component of this position is enhancing recovery and social recreation/rehabilitation skills of clients that will enable individuals to increase their independence, dignity, and self-respect. The role will require direct service and support to clients as well as group facilitation and programing in areas such as housing, daily living, practical assistance, crisis intervention, linkages to other support services, along with program clean up. In addition, the staff member will maintain the established routines of the program including all security, fire and general health and safety practices. **Note**: Crisis services support in the Drop-in program may be required. Occasional evening and weekend and on-call hours may be required.

**416 Community Support for Women** provides a safe and supportive environment for female-identified individuals, who are experiencing or are at-risk of homelessness, and are living with mental health and/ or substance use. 416 Community Support for Women provides daily drop-in (5 days per week), Mental Health and Addiction Case Management, and a Health Clinic. Our aim is to support female identified individuals to improve health and quality of life, reduce isolation while building system knowledge and developing self-awareness.

**What You Will Do**

Through a team-oriented approach, the position attempts to empower the consumer, thereby increasing their independence, dignity and self-respect in a safe and secure environment, by:

- Participating in the intake process for clients referred to the program; conducting an assessment with the individual, help to determine their daily living needs and working together to develop a plan of support and recovery.
- Supporting the personal journey of each individual by building a rapport. Supporting and establishing a plan with each person to meet the daily requirements of their living environment; assisting individuals with acquisition of life skills to increase their awareness of safety issues and for ensuring that residents receive adequate assistance in various aspects of their life.
- Establishing a holistic support network of social, family, spiritual, community medical and psychiatric links and resources, assisting the individual so that the linkages are maintained through non-intrusive follow-up, reminders and by occasionally accompanying clients on their appointments.
- Supporting self-advocacy and advocating on behalf of the client in navigating the social services and health care systems and ensuring accessibility and availability of services; promoting, education and; history taking, screening, referral, and follow up for the health clinic. Support connection to ongoing primary health care services.
- Assisting in the administration of the program by completing and maintaining the required records consistent with case management best practices and standards.
- Engage in the delivery, sorting and distributing of items in the food bank program
- Consult with physicians, nurses, and other health professionals.
- Provide services during regular and extended hours within the clinic and provide outreach services to those unable to access clinic for varying reasons.
- Monitor the ongoing needs of 416 clients attached to the clinic with chronic stable illnesses.
- Work with primary health care and other supports to emphasize health promotion, education, and disease prevention, and recognize the special risk factors (social, physical, psychological etc.) which may affect the health of clients.
