To help fulfill its mission, LOFT is hiring a **Community Support Worker**.

**JOB SUMMARY**:
The Community Support Worker is responsible for providing a range of services in support of individuals in the community who are either HIV positive and/or Trans/Non-binary with addiction issues, mental health and having experienced homelessness. Health stabilization may also be a key challenge.

Key components of the position will be case coordination, building of a support network of community resources that empower individuals and increase their independence, dignity and self-respect in a safe, secure environment. Another component will be the provision of direct assistance in such areas as daily living, crisis management, and health instruction/stabilization. Case co-ordination will also be a component.

**WHAT YOU WILL DO**:

- Participating in the intake process for clients referred to the program
- Supporting the personal journey of each individual by building rapport with them
- Establishing a holistic support network of social, family, spiritual, community, medical and psychiatric links and resources, assisting the individual in maintaining the linkages
- Supporting self-advocacy and advocating on behalf of the client in navigating the social services and health care systems and ensuring accessibility and availability of services
- Encouraging individual participation and participating in house meetings, resident association activities or group activities to promote residents’ wellbeing
- Assisting in the administration of the program by completing and maintaining all required records

**WHAT YOU BRING**:

- Completion of a diploma/degree in Human Services (or equivalent)
- Minimum of three years related experience with the client population
- Knowledge of HIV/AIDS, Gender identity, addictions, homelessness and/or mental health issues
- An understanding of Psychosocial Rehabilitation principles with the ability to operationalise them
- Excellent interpersonal, communication and counselling skills
- Well-developed crisis intervention, problem solving and interpersonal skills
- An understanding of the therapeutic relationship, and an ability to come to an agreement with clients on goals/priorities
- Good time management, analytical and organisational skills
- Knowledge of available community resources and supports
- Ability to work effectively within a team environment and to interact with workers from a variety of agencies
- Understanding of cultural competency and anti-oppression framework
- Reflective of a multi-barriered group, as serviced by the program, a strong asset
- Ability to collaborate and coordinate with multiple formal partners
