**What do you offer?**

The Second Stage Housing Case Manager will work as part of the Housing Services Team that provides programs and services for individuals with affordable/social housing needs. The Case Manager will work with the Director to achieve strategic goals in the fulfillment of the mission of the YMCA of the National Capital Region.

**Some of the tasks of this position include**:

- Providing general information to individuals and agencies regarding eligibility for housing in the Y’s Housing Services and specific information about the Second Stage Housing Program.
- Scheduling and conducting interviews with housing applicants and their supports to assess eligibility and compatibility for the Second Stage Housing Program.
- Developing and maintaining individual goal plans with assigned clients.
- Providing case management, counselling support, crisis intervention, and referrals for clients.
- Providing life and social skills workshops.
- Maintaining standards for reporting and operating procedures.
- Reporting statistical data.
- Ensuring client confidentiality and that all files and records are secure.
- Building and maintaining relationships with relevant agencies and services to promote referrals to the Second Stage Housing Program.
- Maintaining the workplace to ensure a safe and clean environment.

**What qualifications and abilities are we looking for?**

**Education**
- Post-secondary education or equivalent in a related field

**Language**
- Bilingualism in English and French preferred
- Other languages are an asset

**Experience, Skills and Abilities**
- Minimum two years of experience in social services, preferably with youth
- Knowledge and sensitivity in dealing with individuals with homelessness, addiction, and mental health
- Knowledge of community resources relevant to youth
- Current First Aid and CPR certification
- Minimum one year of experience in the housing field preferred
- Training in Suicide Intervention and Non-Violent Crisis Intervention preferred
- Proficiency with Microsoft Office Suite
- Proven facilitation skills for workshops and information sessions
- Strong communication skills
- Strong problem-solving skills
- High degree of initiative and adaptability
- Excellent client service skills and focus
