The Mamisarvik Healing Centre Program delivers Inuit-specific treatment for trauma and substance use disorders. Professional, confidential, non-judgmental recovery and healing services are provided to Inuit 18 years of age or older. Mamisarvik also provides day and residential treatment programming.

**What are we looking for**:
Reporting to the Treatment Manager, the Case Manager will provide ongoing support, advocacy, planning, systems navigation, and service coordination for clients involved in the Mamisarvik Healing Centre Program. The Case Manager can provide crisis intervention and emotional support as needed by the client. The Case Manager is expected to work collaboratively with a multi-disciplinary team of Elders, community members and professionals within Mamisarvik Healing Centre and TI and to establish and maintain partnerships with related community organizations.

**What you will be doing**:

- Conduct comprehensive assessments of client needs; develop collaborative strengths-based and goal-focused safety/care plans; facilitate coordination, communication and collaboration with clients and service providers to help achieve the plan; monitor and evaluate progress towards goals and regularly adjust plans to meet the evolving circumstances of the clients.
- Connect Mamisarvik clients with culturally safe services, support, and resources.
- Occasional provision of short trips with clients and support during other activities.
- Liaise with others to resolve medical, legal, housing, and financial issues that are barriers to participants of the Mamisarvik Healing Centre.
- Advocate for the client and assist them in advancing their rights and overcoming barriers in order to ensure they receive the programs and services they need.
- Provide clients with objective information about processes and options to support them in making informed decisions.
- Provide case consultations with children’s aid societies, transitional houses, Ontario Works, etc. to ensure agency interactions and interventions are done in a culturally sensitive and appropriate way.
- Create positive working relationships and collaborate with other staff at TI and other Indigenous and non-Indigenous agencies to provide integrated and coordinated support for the client to help meet their needs and goals.
- Complete all required administrative duties of the role including (but not limited to) maintaining accurate, confidential client files in our case management system, complete reports and ensure appropriate communication to other MHC staff or team members and with participant.
- Delivering effective services while maintaining professional rapport with clients, peers, and other community service providers.
- Providing referrals to appropriate internal and external service providers.
- Reviewing and evaluating client progress on an ongoing basis and adjusting case plans in consultation with the team and manager.
- May provide on‐call services during treatment cycle.
- Establish and maintain professional working relationships and regular communication with the community and other related organizations.

Comprehensive skills in case management and collaboration, adept with MS-Office and internet, organizational skills, decision-making skills, and excellent communication skills in difficult situations are necessary.
Ability to work independently and within a multi-discipline team environment, to meet deadlines, be discreet and manage confidential information, engage people to build trust and rapport is required.
Bachelor’s degree in social work, psychology or a related field or recognized diploma in human services, social service worker, or related field of study is required. Minimum 2 years’ professional experience in community-based human services including case management is required. Experience working in the alcohol/chemical dependency or mental health field is preferred. CPR, Crisis Intervention and Prevention, Suicide Intervention & Prevention skills and training, Food Handling and Preparation and other training as identified are necessary. Fluency in English is required, fluency in Inuktitut is a definite asset. Valid Driver’s license without restrictions is a definite asset.
