**Forms Coordinator**

The Forms Coordinator plays a vital role in managing the Forms Committee, overseeing the intake, and processing of various forms, maintaining the forms dashboard, and ensuring the efficient production and delivery of forms packages. The role involves analyzing form-related data, coordinating printing processes, and continuously seeking improvements in scanning and forms management procedures.

**Responsibilities**:
**Process Flow, Forms Analysis and Design**:

- Provides consultation, analysis, and design services to develop a range of forms from paper to advanced level ‘smart’ electronic forms by consulting with internal stakeholders and regional partners when required.
- Manages the RVH Forms SharePoint site and forms repository including future dashboard analysis and maintenance.
- Develops new forms and/or modifies existing forms in accordance with RVH policy and industry standards. Maps patient demographic data and barcodes the forms and labels using Planet Press. Evaluates process flow to develop new avenues of efficiencies and improvements for patient documentation. Utilizes procedural analysis techniques to document and optimize workflow, then integrate with relevant software.
- Tests and verifies forms against business requirements and validates usability of forms with end-users. Participates in projects offering expertise in forms creation and management.

**Forms Committee**
- Coordinates meetings of the Forms Committee, ensuring representation from relevant departments. Facilitates discussions, gathers feedback, and implement decisions regarding form updates, revisions, and archiving.

**Forms Rooms Management and Production**
- Coordinates with internal and external resources to produce required forms in a timely manner. Ensures quality control in printing processes and compliance with RVH best practices and other clinical standards as required. Compiles forms based on specific department requirements. Organizes and coordinates the delivery of forms packages to designated recipients.

**Downtime Support**:

- In collaboration with Professional Practice, assist with preparation, maintenance, and production of down-time forms.

**Qualifications**:
**Education**
- 2-year College Diploma or equivalent required

**Experience**:

- Minimum of one year experience in a forms and printing production setting
- Experience in work related to data assurance metrics, statistics, and analysis
- Experience with teaching and training
- Previous experience in forms management, document control, or related administrative role within a healthcare setting preferred.
- Experience with SharePoint, Microsoft Suite, Adobe, PlanetPress, Plexxus

**Competencies**:

- Proficient in multiple software platforms for forms creation, distribution, and retention
- Ability to review existing processes and re-engineer to achieve the maximum utilization of services
- Strong communication skills
- Ability to deal with stress and demanding situations
- Strong knowledge of forms design drafting and architecture
- Exceptional communication and interpersonal skills for effective collaboration with diverse stakeholders.
- Ability to work independently and manage multiple priorities in a fast-paced environment.
