Community Living Burlington is a social-profit charitable organization serving the communities across the Halton region since 1955. Our mission is to enrich the quality of life of people with a developmental disability and promote their full and meaningful inclusion in the community.

The Next Step Connect program aims to facilitate seamless transitions for individuals who have a developmental disability from high school into further education, volunteer opportunities, day services, and ultimately, employment services. As the Connections Coordinator for the Next Step Connect program, you will play a pivotal role in collaborating with Burlington high schools, particularly those implementing the Community Pathway Program, to provide tailored support and guidance to students and their families as they navigate post-graduation transitions.

Please note: This job description outlines a three-year timeline for program activities and improvements, with the expectation that the Next Step Connect Coordinator will adapt and evolve strategies based on ongoing evaluation and feedback. The position is a three-year contract with potential of renewal.

**Responsibilities**

**Year 1**:
Program Launch and Engagement:

- Officially launch the Next Step Connect program, establishing its presence and objectives within the community.
- Actively participate in the Halton Transition Advisory Committee to stay abreast of relevant developments and collaborate on transition initiatives.
- Engage with targeted Burlington high schools offering the Community Pathway Program to build rapport with students, families, and educators.
- Collaborate closely with schools and families to understand individual student transition plans and needs.

Relationship Building and Partnership Development:

- Develop and maintain relationships with existing Employment Services businesses/organizations to enhance support opportunities.
- Connect with new local organizations and businesses to explore potential partnerships and broaden support networks.
- Make recommendations based on individual student needs and aspirations, ensuring personalized transition plans.

Workshops and Internship Facilitation
- Plan and implement monthly peer-to-peer meetings/workshops to foster skill development and community engagement.
- Organize various skills/trades workshops with hands-on community experiences, identifying and inviting community experts for specialized sessions.
- Coordinate internship placements with community businesses/organizations, facilitating participants' understanding of expectations and ensuring a smooth experience.
- Work alongside participants and community partners during internships, actively supporting engagement and collaboration.
- Collect feedback and recommendations for program enhancement, ensuring continuous improvement.

**Year 2**:
Evaluation and Adjustment:

- Evaluate Year 1 outcomes and gather feedback from participants, schools, and community partners to inform program adjustments.
- Adjust program elements based on feedback to enhance effectiveness and address any identified areas for improvement.
- Strengthen relationships with existing Community Pathway Programs and local organizations/businesses, identifying opportunities for deeper collaboration.
- Enhance Participant Engagement and Support:

- Introduce new features or improvements to the Next Step Connect program based on feedback and evolving needs.
- Expand the variety of workshops and courses offered, planning advanced or specialized training opportunities for participants.
- Conduct mid-program evaluations to measure progress and gather feedback, strengthening relationships between participants and community partners.

**Year 3**:
Continued Evaluation and Growth:

- Evaluate Year 2 outcomes and identify areas for improvement, particularly focusing on the effectiveness of the internship component.
- Analyze outcomes, impacts, and participant success stories to inform program evolution and expansion.
- Introduce new topics or specialties to meet emerging trends and further enhance participant experiences.
