Health Sciences North (HSN) is seeking an innovative leader for the temporary position of Coordinator, NEO Navigation to co-ordinate, establish and manage a referral and intake process within the NEO Kids and Family Program and support the review of the intake process within the Mental Health and Addictions Division’s Psychiatric Outpatient Clinic. The Coordinator will undertake project activities including research, planning, coordination, community engagement, communication and provide the information and connections needed to establish the structure and function of a new referral and intake process within NEO Kids.

**KEY FUNCTION**:
Co-ordinate, establish and manage a referral and intake process within the NEO Kids and Family Program and support the review of the intake process within the Mental Health and Addictions Division’s Psychiatric Outpatient Clinic. Undertake project activities including research, planning, coordination, community engagement, communication and provide the information and connections needed to establish the structure and function of a new referral and intake process within NEO Kids.

**REPORTING**:
Under the general direction of the Administrative Director, NEO Kids and Family and Inter-Professional Practice.

**DUTIES**:

- Lead the development, implementation, and ongoing maintenance of NEO Navigation, the SmartStart Hubs and the new electronic referral process.
- Lead the implementation and ongoing maintenance of the new electronic platform.
- Identify current referral package components and potential future opportunities, considerations, revisions.
- Manage human resources which include: recruitment, selection and training.
- Guide, motivate, monitor and supervise staff, develop standards of performance, evaluate performance and make recommendations on disciplinary action as required.
- Organize, plan, develop, implement, manage, and evaluate NEO Navigation/SmartStart Hub activities and policies relating to improvement initiatives.
- Map out future state referral and navigation pathways for pediatric services.
- Support the implementation of the NEO Navigation, SmartStart Hub and Ocean eReferral processes.
- Develop annual budgets and assume fiscal accountability of the departmental operation according to approved budget.
- Collaborate with the NEO Kids and Mental Health and Addictions Division leadership team to help implement OCEANs.
- Provide education and training to intake staff around OCEANs and the new referral process.
- Communicate/provide presentations on the new central intake program and new electronic referral system to all key stakeholders.
- Maintain waitlists and database for pediatric referrals.
- Develop standards of performance, evaluate performance for NEO Navigation and make recommendations as required.
- Oversee the work of others in compliance with the Occupational Health and Safety Act (OHSA), its regulations, and HSN policies.
- Teach, coach and lead staff in the development and monitoring of quality indicators and improvement initiatives.
- Collaborate with internal and external stakeholders/organizations/community groups to achieve program objectives, integrate and link services and foster partnerships across the continuum of service delivery.
- Ensure a safe environment for patients, staff and visitors; investigate report, debrief and take or direct corrective action as required on incidents.
