**KEY FUNCTION**:
Process referrals received for NEO Kids Children’s Treatment Centre (CTC) Services, inclusive of SmartStart Hub Services. Conduct intakes and provide timely supported connections to the appropriate community services and supports that best meet the client and family’s needs.

**REPORTING**:
Under the general direction of the NEO Navigation Coordinator, NEO Kids & Family Program.

**DUTIES**:

- Process new referrals (received via Ocean eReferral Network) for NEO Kids CTC Services, inclusive of SmartStart Hub services.
- Review accuracy of referrals received and collaboratively work with referring providers and clients/families; triage/prioritize referrals that are eligible for CTC services.
- Conduct exploratory conversations with clients/families and perform comprehensive intakes.
- Identify and facilitate connections to appropriate services and supports both within the NEO Kids/CTC programs and community agency partners based on client’s needs and eligibility for service; enable immediate access to relevant resources.
- Provide current information related to services and supports available at the CTC and in the community.
- Consult with regulated health professionals (i.e. speech and language pathologists, occupational therapists, physiotherapists, community agencies, etc) from the child development sector as required.
- Maintain accurate and timely documentation and reports, collect relevant data and update client records within electronic medical record (EMR) system.
- Respond to inquiries from families, service providers, NEO Navigation team, Children’s Treatment Centres, schools and other community partners.
- Perform waitlist management activities.
- Contribute to ongoing program development and evaluation and assist in the implementation of new practices as required.
- Support and contribute to the implementation of NEO Kids’ regional centralized access point through Oceans eReferral Network.
- Participate in program training and education as required.
- Evaluate patient care and satisfaction on an ongoing basis and report back to leadership.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.

**QUALIFICATIONS**

**EDUCATION AND TRAINING**:

- Minimum of a two (2) year Diploma in a related health care field from an accredited college.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

**EXPERIENCE**:

- Minimum of two (2) years’ experience working in a pediatric environment.
- Experience working with data management software is preferred.

- Demonstrated knowledge of current trends in children’s rehabilitation services and relevant community services and supports for children with special needs and their families.
- Demonstrated knowledge of child development and paediatric conditions.

- Demonstrated knowledge of Medical Terminology.
- Demonstrated ability to prepare, write and maintain reports/records.
- Demonstrated excellent presentation, communication and organizational skills.

- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
- Demonstrated discretion and maturity when handling confidential information.
