**Durham Community Health Centre (DCHC)**, formerly known as Carea Community Health Centre, is a registered, charitable organization that provides integrated, accessible, and equitable community based primary care, wellness services, and health education to Durham community members who face multiple barriers to their health and well-being.

DCHC also provides equity-based programs and services that focus on priority populations such as Indigenous, Black, the 2SLGBTQI Community, Newcomers to Canada, Seniors, and Unattached patients (i.e., those without a family doctor), to name a few. DCHC ensures Durham community members receive not only sick care, but preventive care as well.

At DCHC, we are a team-based interprofessional group of staff including physicians, nurse practitioners, nurses, counselors, dietitians, outreach workers, medical secretaries, and other administrative staff. We always place every client at the center of our approach to care, based on their needs. We address these needs through integrated clinical and wellness care and health education.

**Durham Ontario Health Team (DOHT)**

The Durham Ontario Health Team (OHT) is a collaborative group of organizations, primary care providers and patient, family and care partner advisors working in partnership to improve access and delivery of coordinated health services. The group is comprised of partners from across the care continuum, including primary care providers, hospitals, mental health and addiction services, social support services, home and community care support services and long-term care.

The Ministry of Health (MOH) announced the province’s first OHTs in December 2020. The Durham OHT was among this group. In September 2023, the Durham OHT was one of the twelve OHT’s chosen to accelerate and become a Designated OHT.

While the core Durham OHT partnership group is comprised of 18 signatory organizations, there is broad support from more than 70 health care partners across Durham Region, representing various organizations in the health, social, education, and private sectors.

**Position Overview**

In this unique and exciting role, the incumbent will be supporting DCHC and DOHT collaborative partners that serve health and social care in the Durham Region. As part of the Development Team, the Community Engagement Specialist position includes relationship building, partnership development and maintenance and coalition building.

**DCHC - Development (50%)**
- Working closely with AVP and Manager, Development and Stakeholder Relations to provide support for the delivery of DCHC’s annual fundraising program. Ability to demonstrate professionalism and confidentiality with regard to donor data including possible program impact
- Coordinate event logistics in particular the annual Golf Tournament, Annual General Meeting, etc.
- Manage the Third Party, and Tribute giving (gifts in honour/memory of) portfolios
- Work collaboratively on strategies to increase the level of engagement of donors and sponsors
- Co-ordinate and support Development team including data analysis and report writing, preparing lists and mailings, organizing, and communicating professionally with donors at all levels
- Maintain current records in CRM database including administrative duties such as tax receipting, record updates, lists and more.

**DOHT - Engagement Administrator (50%)**
- Working with the Director, Durham OHT, create meeting agendas, manage action logs and communicate with all Partners of the Durham OHT
- Provide administrative support to the Primary Care Advisory Council, Patient Family and Care Partner advisory council by attending all meetings and recording meeting minutes
- Assist with sharing local health resources to increase Durham’s regional health system literacy
- Liaise between community stakeholders and Council Co-leads
- Provide leadership and support to all community events
- This role will require flexibility in schedule and in person support.

**Qualifications**:

- University degree and/or College diploma in relevant discipline or equivalent/relevant experience in community engagement, event management, preferably in the charitable sector
- A minimum of 2 years experience leading stakeholder engagement across a range of sectors in health and social services, and building strong strategic and productive relationships
- Demonstrated project management experience, including the ability to liaise with internal stakeholders, vendors, determine priorities, manage workflow and deliver quality product on time and on budget
- Must demonstrate experience working with and utilizing confidentiality and discretion
- Ability to work well under pressure in a fast-paced environment and be adaptable to the changing needs of the organization
- Strong problem-solving skills and attention to detail and accuracy with ability to multi-task, organize, prioritize, and meet deadlines
- Excellent interpersonal and collaboration skills with a proven ability
