Youth Wellness Hubs Ontario is a multi-service hub for youth aged 12-25 in Rockland and Cornwall. These sites provide rapid access to services that address the needs of youth related to mental health, substance use, primary care, employment/training, housing and other community and social services. This initiative, led jointly by the Cornwall Community Hospital, Valoris for Children and Adults of Prescott-Russell, and Hawkesbury & District
General Hospital includes over 20 partner organizations.

**WHAT TO EXPECT?**
The Care Coordinator uses a client-centered approach to meet the needs and expectations of young people while building the capacity of families and service providers to provide care. He or she works in collaboration with all members of the team, ensuring clear communication between all stakeholders involved in the care of youth. The Care Coordinator does not only assess immediate needs, but all clinical needs as well. They also provides outreach services to ensure the client is referred to services that meet their needs, to facilitate access to services for rural youth and to support successful youth transition planning.

**QUALIFICATIONS**:

- University degree from a recognized university in social work, community health, or health science, or a combination of a post-secondary degree and extensive experience
- One to three years of experience in community health or mental health and addictions, preferably with youth
- Experience working with the public, an interdisciplinary team, committees, as well as community partners
- Knowledge of the realities concerning young people today, especially in terms of mental health and addiction
- Knowledge and mastery of current assessment techniques, short-term therapy, narrative approach and training in non-violent crisis intervention
- Ability to establish and maintain relationships with youth, families and community organizations and groups
- Ability to promote a community engagement approach by providing the appropriate information and tools to the target clientele and to work in collaboration with various professionals, including peer helpers
- Ability to act as a resource person for young people and the Carrefour team and to provide the necessary support in achieving their objectives
- Effective interpersonal relations, concern for customer service and team spirit
- Excellent ability to communicate in French and English orally and in writing
- Have a driver's license and access to a car and have received the vaccination against COVID-19 as recommended by public health
