**Position Summary**

The Inter-professional Primary Care Team (IPCT) will work collaboratively to provide primary care services to registered Primary Care clients and community clients, in various settings (at the CHC, on the CHC community health bus, at partner agencies or outdoors in the community) with a specific focus on marginalized populations. The IPCT team will provide services such as access to chronic disease prevention and management, mental health and addiction assessment, referral and counselling, as well as primary care clinics and outreach primary care services.

We are seeking a Social Worker/Psychotherapist to join our Inter-professional Primary Care Team (IPCT). This role provides direct counselling to individuals, couples, families and provides groups for clients with both walk-in and appointment availability. This position will also provide counselling groups including both psycho-educational and therapeutic groups. This position will provide services to community clients with a focus on vulnerable and marginalized populations using both a walk-in and appointment based model. The IPCT will work closely with primary care providers and community partners in Brantford, Brant and Norfolk.

The Social Worker/Psychotherapist works collaboratively with members of the IPCT to plan, implement, coordinate and evaluate the programs and services provided to clients. The Social Worker/Psychotherapist will provide individual services, clinics and group programs as needed. This position will support the IPCT by conducting assessments to develop counseling care plans and that support the overall treatment plan for the client. The Social Worker/Psychotherapist will also work individually or with others on the team or in the community to provide education and support for mental health and addictions and chart according to college requirements in an EMR. The Social Worker/Psychotherapist will support the evaluation process for this program.

**Qualifications**
- Masters degree in Social Work/Psychotherapy or equivalent from a recognized university
- Registration with the Ontario College of Social Workers and Social Service Workers or College of Registered Psychotherapists of Ontario
- 3-5 years experience in a community setting working with marginalized populations.
- Knowledge, training and experience in different models of counseling, assessment and psychotherapy
- Knowledge of and experience with group facilitation and co-facilitation
- Experience working with electronic medical records (EMR).

**Responsibilities & Skills**:

- Provide counselling support and group programs for clients with a range of mental health needs in a primary care setting who may be experiencing barriers to care such as no primary care provider or barriers to health that include the social determinants of health.
- Provide primary care and mental health support through a relationship and community based framework
- Work collaboratively with the team and other system partners (medical providers, shelters, hospitals, other community teams supporting similar clients) to develop, coordinate and execute care plans
- Demonstrated knowledge of mental health, concurrent disorders, treatment modalities, and harm reduction practices is required
- Strong communication (verbal and written), organization, and time management skills
- Demonstrated ability in conflict management and interpersonal relations
- Understanding of anti-oppression principles and ability to work effectively with people of various cultural, age and economic backgrounds
- Ability to work collaboratively within and across teams, developing and maintaining positive relationships with service providers
- Ability to work independently and in self-directed working conditions
- Able to work flexible hours primarily within a 5 day, Monday to Friday work week. Regular evening work 1-2 times per week.
- Able to travel within the Brantford-Brant-Norfolk OHT area
