- We are seeking compassionate and enthusiastic Personal Support Workers to join our organization. You will be an integral part of a collaborative and inter-disciplinary team focused on delivering exemplary care to our clients_

Reporting to the Director Long Term Care (or delegate), the Personal Support Worker (PSW) utilizes the Enhanced Living Model to assist residents with personal care and activities of daily living in the assigned neighborhoods by following the individualized care plans.

**ESSENTIAL DUTIES AND RESPONSIBILITIES**

Under the direction of the Registered staff, the PSW assumes the responsibility for providing direct care to the resident and documenting such care according to the care plan in the following areas:

Performs all duties in accordance with the Mission Statement and policies and procedures of SJLCB as adopted from the Ministry of Health guidelines.

- Encouraging and supporting individuals’ families to participate in care

The Provision of Resident Care
- Assisting with personal hygiene (bathing, dressing, hair care, mouth care, nail care, and shaving)
- Assist with nutrition (providing fluids and nourishments as required, meal set up, assistance with feeding, and reporting of intake. Weights to be taken monthly and changes to be reported immediately.)
- Assisting with urinary and bowel elimination (toileting, disposable briefs, emptying of urinary and colostomy drainage bags, and reporting of output)
- Promoting comfort by assisting with positioning, mobility, and ambulation as per care plan (positioning and turning of residents; using mechanical lifts appropriately; and reporting of any changes)
- Assisting with exercises
- Facilitating appropriate rest and socialization
- Establishing and Maintaining Healthy Interpersonal Relationships
- Demonstrating enthusiasm for working with older adults and their families
- Developing and preserving therapeutic PSW/resident relationships
- Developing and preserving collaborative relationships among members of own nursing team and the healthcare team
- Developing and preserving collaborative relationships with other programs/departments

Ensuring the Quality of Health Care Practices
- Contributing to the quality of working life of peers and colleagues
- Improving the quality of life of residents through the ‘enhanced living model’ of practice
- Learning and using new concepts and approaches in the care of older adults
- Participating in improving the care provided to older adults
- Consulting with peers and other health providers in approaches to the care of older adults
- Contributing to and utilizing the established plan of care for the residents

Maintaining Professionalism and Work Role Competencies
- Attending and participating in educational in-services
- Showing an interest in the profession by projecting a competent, professional image through attitudes, actions, and appearance
- Demonstrating professionalism by being dependable in attendance and punctuality
- Provide services in a caring, respectful, and cost-effective manner
- Participating in research and continuous quality improvement activities for improving the care provided to older adults
- Recording documentation in an accurate and timely manner according to policies and procedures
- Contributes to a transparent culture of client and staff safety by adhering to and abiding by patient and staff safety policies and procedures set by SJLCB
- Promotes an environment that encourages and supports change using change leadership theory

**QUALIFICATIONS**:

- Must possess a Personal Support Worker certificate or diploma from Ministry of Health of Ontario, Long Term Care Division approved educational facility.
- The PSW must have strong organizational and prioritization skills as well as demonstrated teamwork skills.
- The PSW must possess a clear understanding of ethics
- Demonstrated written and verbal communication skills as well as excellent interpersonal skills
- Excellent problem solving and conflict management skills are essential.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to stand, walk, reach with hands and arms, stoop, kneel or crouch. The employee must regularly lift and /or move up to twenty five (25) pounds.
