Position Summary

Effective September 7th, 2021 all applicants must provide proof of full vaccination against Covid-19 (in the form of watermark receipts from the provincial portal) or provide written proof of a medical or religious reason for not being fully vaccinated as per the provincial direction as set out by the CMOH Directive 6.

**Education Requirements**:
Masters of Social Work Degree (M.S.W.) from an accredited University School of Social Work (preferred)
Registration with the Ontario College of Social Work and Social Service Workers (OCSWSSW)

**Mandatory Qualifications**:
Recent Social Work experience in a hospital, including acute Medical and post-acute programs and discharge planning for populations listed above.
Demonstrated competency in providing services using an anti-oppressive approach as a member of an interdisciplinary team.
Proven ability to manage competing demands and prioritize work in a time-sensitive environment.
Excellent working knowledge of related provincial legislation including Health Care Consent Act, Substitute Decisions Act, Public Hospitals Act, community resources
Working knowledge of social supports including, Ontario Works, Ontario Disability Support Program (ODSP).
Knowledge of psychosocial, relational and health issues across the life span, including family birthing, paediatrics, grief and loss, caregiver stress/burnout, chronic illness and psychosocial/physiological conditions affecting the patient population
Proven ability to work in an empathic manner to engage hard to reach and marginalized populations
Evidence of ongoing professional development with respect to the practice of social work
Excellent written, verbal and interpersonal skills
Excellent attendance record
