**Job Summary**:
In accordance with the Mission, Vision and Values, and strategic directions of PHSA, safety, including both patient and employee safety, is a priority and a responsibility shared by everyone at PHSA. As such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

The Coordinator, Quality, Safety & Outcome Improvement reports to the Director(s), Quality, Safety and Accreditation and is actively involved in the planning, development, implementation and evaluation of quality improvement initiatives and maintains primary coordination of quality initiatives for the Director(s) and team with a person-centred approach related to quality and safety. This includes the broad areas of knowledge exchange, information dissemination, and education for internal and external stakeholders. This role is responsible for supporting the Director(s), Quality, Safety and Accreditation with accreditation readiness and sustainment including coordinating staff working groups, mock surveys, the Accreditation survey work plan, and on-site visits and supports the communication of results to leadership and relevant stakeholders. The Coordinator identifies, collects and analyzes information and metrics related to quality initiatives and patient safety reviews and incidents; reports findings regarding the implementation, sustainment and effectiveness; and provides related input and suggestions based on the data, best practices and current research to the Director(s). This role compiles the quality and safety action plan and an annual report, in collaboration with the Director(s) and program teams, to communicate and highlight quality improvement initiatives for the program.

**Duties/Accountabilities**:

- Coordinates the execution of quality initiatives that improve patient safety standards by drafting project charters, process maps, and timelines for the team using provided information; maintaining action logs; providing key stakeholders with status updates regarding quality initiatives; and following up with team members regarding meeting action items and tasks to confirm completion within the required timelines, adherence to project protocols, and conformance to applicable institutional and ethical policies and procedures and privacy legislation. May include supervision of staff or students assisting in large or complex quality initiatives.
- Evaluates quality initiatives by identifying, collecting, and analyzing relevant information, including outcome and process metrics related to current quality initiatives and patient safety reviews and incidents to confirm implementation, sustainment and effectiveness. Reports on findings and provides input and suggestions based on the data, best practices and current research to the Director(s) and quality initiative/program team for development, implementation, analysis, dissemination of findings, and policy/practice implications.
- Actively supports the Director(s) with the management and facilitation of time-limited quality initiatives and accreditation readiness and sustainment, including obtaining staff input and organizing working groups with relevant stakeholders as indicated by the Director(s) to collaboratively determine priorities, identify better practice strategies and activities for health professionals, and develop and implement policies and procedures to improve quality, safety and patient experience and meet Accreditation Canada and other accreditation agency standards as appropriate.
- Coordinates and prepares for meetings through methods such as drafting agendas, ensuring reports and tracking tools are pre-circulated, records meeting minutes including the action items, associated responsible person and timelines for tasks, and progress is reported at subsequent discussions. Drafts and implements communication plans and compiles written reports on results to inform leadership and relevant stakeholders regarding quality initiatives.
- In collaboration with the Quality Leaders and/or Clinical Leads, tracks the progress and completion of approved recommendations resulting from patient safety event reviews.
- In collaboration with the Director(s) and program teams, compiles the quality and safety action plan and an annual report to highlight quality improvement initiatives for the program. Works with the Director(s) and Quality Leaders to develop, implement and analyze evaluative surveys and special projects to support the development of the quality and safety action plan and quality improvement initiatives.
- Develops and maintains a database that conforms to accreditation standards and measures to track data such as recommendations related to patient safety reviews. Analyzes the data from patient safety reviews to identify data trends, such as common gaps across departments, to provide notification and suggestions to the Director(s) and quality improvement/program team.
