**What will you do?**

You will act as the central point of contact for our clients and their support network, from intake to completion of service. You will help them navigate our Employment Resource Centres and assist them in any employment related requests.
- You will be responsible for the following processes and procedures:

- communicate and orientates client to Prospect and program services
- schedule and coordinate intake meetings with clients and members of their support network
- develop and complete assessments of clients’ skills, abilities and needs
- schedule and facilitate development of client Learning Plan and goal accomplishment indicators, including timelines for accomplishment
- develop employability assessments (as applicable)
- Maintain knowledge of supports and resources within the community, including financial supports, funders, mental health service providers, supports for persons with developmental disabilities, crisis supports, behavioral supports, counseling supports and education and training supports
- Complete timely maintenance of client file(s)/documentation, including but not limited to:

- assessments, client learning needs and style
- learning plan and appropriate progression indicators
- Make required updates and communicates changes to others on the Prospect team and externally, as required
- Support other staff working directly with Prospect clients; providing applicable support to teach and expand identified literacy and employability skills
- Contribute to/completes all contractually required documentation
- Participate in program planning, service design, development, and delivery in accordance with client goals, Prospect policies and procedures and FOIP
- Prepare for and facilitate various literacy specific programming with clients
- Support the creation, progression and completion of goals for clients
- Maintain strong working knowledge of adult literacy standards and education principles

**What does it take to do this job?**
- 1+ years of experience in teaching / facilitation
- 1+ years of experience in working with individuals with developmental disabilities
- Experience in curriculum development and creation of lesson plans
- Familiar with assessment tools
- Intermediate knowledge of Excel, Word, and PowerPoint
- Advanced knowledge of Outlook and Internet
- Strong oral communication skills to be able to relay instructions
- Exposure and/or direct experience with health and safety and risk management practices as it relates to activities
- Proven ability to work in a team environment and individually (when required)
- Demonstrated Ability to coach and mentor
