Follow Up Support Worker

The Northern Alberta YMCA is looking for a result focused Follow-Up Support Worker to implement and support the execution of the Housing First Philosophy program to individuals, couples and families who are chronically homeless with multiple barriers and high needs. The Follow-up Support Worker is accountable to the YMCA team and participants as a whole to ensure successful program delivery.

**Position Res**
**ponsibilities**
- Build relationships with staff, volunteers, and participants with regular communication.
- Assist with volunteer recruitment.
- Offer participants high quality services; ensure that staff and volunteers are also delivering services matching YMCA Standards.
- Contribute to building the team by assisting in orientations and training and by providing feedback and appreciation to staff and volunteers.
- Plan, implement and evaluate programs within YMCA guidelines and contracted performance measures.
- Recommend quality improvement ideas and implement improved changes to programs and services.
- Build and maintain relationships with landlords, resident managers and other community supports.
- Communicate with participants and provide intensive case management support.
- Visit participants weekly to support them in maintaining their housing, improving life skills and help them build a connection to their community.
- Develop and monitor Individual Action Pland and work with participants to meet their goals.
- Effectively communicate the progress of participant as needed.
- Complete all administrative duties, including but not limited to; maintaining participant files, compiling monthly statistics, quarterly outcome reports; as needed accurately and in a timely manner.
- Work within a budget, and maintain claims forms and tracking sheets. Communicate information to contribute to forecasting and budget activities.
- Establish community relations relevant to the program, network with other providers of Housing First Services. Provide information about service to internal and external stakeholders.
- Create and share success stories as appropriate.
- Work as a flexible team member throughout the facility.

**You’ll**
**Need for This Job**
- University Degree in Human Services or related education
- Annual registration in a professional organization as required by law
- 2 years of related experience
- Valid driver’s license and vehicle.
- Non-Violent Crisis Intervention and Suicide Intervention Certification are preferred
- Certifications: Standard First Aid & CPR-C
- Able to obtain a Police Information Check and Vulnerable sector and Child Intervention/Enhanced Security check
