About The Role:

The Encompass Service Coordination Program within WIllowbridge offers individuals living with an intellectual disability the support they need to access safe housing, medical, financial, or mental health services and to develop socially valued roles within their community. We are looking for a high performing Service Coordinator with a commitment to service excellence to join the Encompass Team for the summer. This role would be suitable for a 2 nd year BSW student.

The Service Coordinator is responsible to work with individuals, caregivers, families, and a network of support agencies to ensure that children under 18 and adults living with developmental disabilities or a dual diagnosis receive adequate care, support, and access to fundamental services, ensuring the provision of trauma and attachment informed treatment that is welcoming and safe, and where our clients and their families can find solutions to help reach their potential and support their goals of participating more fully in their community through work, play and friendship.

  • – Regular meetings with clients in the community to coordinate and support their access to services they require.
  • – Develop and implement individual support plans that are lead by the individual with a person-centred approach.
  • – Complete regular reports, updates and evaluations, documenting partnerships, programs, and service coordination.
  • – Ensure referrals are made to the appropriate internal partners, or community agencies that are best suited to support vulnerable individuals or families using a holistic, trauma informed approach.

What We’re Looking For:

  • – Working towards completion of second year of a Bachelor’s Degree in Social Work (BSW) or equivalent.
  • – Work and/or practicum experience providing case management services or specialized clinical supports; preferably working with individuals with a developmental disability or with individuals who have multiple complex needs and/or a dual diagnosis.
  • – Proven knowledge and experience collaborating with community services.
  • – Ability to maintain a high degree of confidentiality and exercise sound judgment.
  • – Excellent organizational and time-management skills.
  • – Ability to maintain composure in challenging situations and when presented with multiple demands.
  • – Proven ability to work well independently and as part of a team.
  • – Ability to communicate with clients, staff, volunteers, and the public in a respectful, cooperative and professional manner.
  • – Excellent communication and interpersonal skills.
  • – G driver’s license and access to a reliable vehicle is considered an asset.
