Position Title Client Care Navigator Location Calgary or Edmonton, AB Term Full‑time, Contract Reports To Manager, Healthcare Operations Salary $25.88/hour Posting Date November 6, 2025 Closing Date November 20, 2025 I. Scope And Summary Of Primary Responsibilities The Client Care Navigator (CCN) will coordinate and integrate supports and services around the person living with vision loss. In this direct client service role, the CCN will be the key contact or navigator for individuals after a vision loss diagnosis. Responsibilities include assessment, supporting the client to achieve their care plan goals, and strengthening communication and linkages between VLR staff and external community health care providers across the continuum of care. To provide individuals with vision loss, healthcare navigation to increase and enhance the healthcare system's capacity To improve the client's experience and wellbeing To enhance a client's capacity to remain independent for as long as possible II. Essential Functions This position entails, but is not limited to, the following general responsibilities: Schedule all VLR therapy appointments with primary and specialised service providers Seek feedback on client satisfaction and take remedial action as required Monitor and provide proactive follow‑up for clients to ensure ongoing collaboration across services and providers Identify opportunities for new or emerging care options to meet changing needs and to address service/support gaps Ensure that all service‑related activities are documented in the client health record following VLRC policies Interact with clients, their family, and caregivers to develop and identify vision‑related goals Build and maintain good working relationships with professionals in other agencies and community groups Educate community partners about Vision Loss Rehabilitation Canada and its specialised services Participate in multi‑disciplinary team meetings Pro‑actively facilitate linkages, communication, information exchange opportunities, and coordination between clients and service providers in the health care system Understand roles and linkages between the primary and specialised service provider with the service delivery model Build and/or maintain partnerships through sharing knowledge, treating confidential information appropriately following VLRC policies when dealing with partners and stakeholders Demonstrate experience in caseload management, medical documentation, and maintain electronic medical records Requirements III. Qualifications Knowledge And Skill Requirements Understanding of roles and linkages across primary care, community care, and specialised geriatric services Knowledge of client‑centred philosophy related to interviewing, assessments, care planning, and scheduling practices Clinical expertise related to client‑centred interviewing, assessments, and scheduling practices Experience in client assessments and care planning/coordination Excellent communication skills (verbal and written) Strong interpersonal skills, including shared decision‑making and facilitation Ability to prioritise workload, manage competing tasks, problem‑solving skills and resourcefulness Proficient in technology (e.g., Microsoft Office and case management and care coordination systems) Ability to work independently and within a team Experience And Education Client service experience in the health and/or social service sectors Diploma in Social Services Work, Community Work, or Mental Health Experience working with individuals with vision loss Patient Navigation certificate would be an asset Experience in cultural sensitivity and diversity would be an asset Work Environment Willingness to work flexible hours Willingness to work in a variety of settings Benefits At VLRC, your work has real impact.
