The Healthcare Navigator position will assist individuals formerly in the justice system and willing to desist from criminal behaviour in the Edmonton community. The Navigator will be an Licensed Practical Nurse or Primary Care Paramedic and will support and work with the members of the Edmonton Police Service (EPS) Integrated Offender Management (IOM) Unit of the Diversion and Desistance Branch to help individuals navigate and connect with the wider health care system and collaborate with clients and EPS to develop and support individualized care plans.

**Description of Duties and Responsibilities**

**Client Case Management**
- Collaborate with the IOM/OM team by identifying risks and intervention responses to reduce recidivism.
- Integrate the Unit’s mission and vision into client-centered case management plans that foster coordinated and timely services that meet the client’s basic needs.
- With community professionals and agencies, develop effective safety and support systems for clients and the public.
- Engage with incarcerated clients on reintegration plans to ensure smooth transition back into the community.
- Remove barriers to client care by identifying critical resources for clients, helping them navigate health care services and systems, and promoting client health, then following up on task completion. Report any gaps in service to Sergeant for further review.
- In collaboration with assigned EPS Detective, implement social case management decisions related to the client’s addictions treatment, mental and physical health status while following basic risk-need-responsivity principals and balancing client and public safety needs.
- Develop and maintain a network of medical and paramedical community service providers.
- Provide basic first aid and wound care, and naloxone training when appropriate.
- Support crisis intervention for a client in distress and report immediately to Detective or Patrol.
- On occasion, assess and address client/offender personal and family crisis both inside and outside of regularly scheduled work hours.

**Documentation, Reporting and Meetings**
- Attend daily meetings to discuss client status and provide timely client updates to Detectives.
- Report to Diversion and Desistance Admin Sergeant and Team Lead on daily activities and whereabouts.
- Maintain accurate and adequate records of assessments, services rendered, and client outcomes on agency and EPS systems, in keeping with GSS and EPS file expectations.
- Provide regular reports as required.
- Attend agency directed training and meetings and represent Diversion & Desistance Branch at meetings as needed.

**Position Qualifications**
- Degree of Diploma in social or human services or related field, or a minimum of 3 years relevant experience working with complex, high-risk populations. In good standing with any relevant professional bodies
- Strong leadership, teamwork, group facilitation skills, and effective interpersonal and communication skills. Open, accepting, and non-judgement attitude with an ability to build rapport with clients
- Experienced at individual and team conflict resolution
- Strong understanding and knowledge of community resources and issues faced by high-risk individuals with a significant trauma experience and high criminogenic factors, and now experiencing homelessness, poverty, and mental health conditions and substance use
- Experience working with and understanding the unique challenges that Indigenous and multicultural individuals and families face, particularly those with significant criminal histories
- Passed an Enhanced Security Clearance through Edmonton Police Service
- Completed training in Suicide Intervention, First Aid, CPR, Crisis Intervention and De-escalation
- Valid driver’s license, registration, and insurance, for personal vehicle
- Strong digital literacy skills
