Program Overview – Seniors Active Living Centre & Outreach (SALCO), Community Care Unit
The WoodGreen Seniors Active Living Centres & Outreach portfolio is composed of several programs including:
WoodGreen’s Seniors Active Living Centres (SALCs) offer health promotion and wellness programs, educational workshops, community engagement activities, cultural events, outings, as well as fall prevention and exercise classes helping seniors stay healthy, active, and independent.
The Outreach to Diverse Seniors program within SALCO funds grassroots groups and communities to deliver programs for seniors to help them reach their health goals, and enable them to age in community. Grassroots groups deliver programs focused on health promotion, outreach, and connection to existing services. The goal of this program is to provide funding and oversight to grassroots groups to address gaps in health service provision and work towards equity in accessing health services in a community setting.
Community Active Response for Elder Support (CARES) Project and Social and Safety Visits program is consists of two services:

  • The Friendly Visiting Program is a support service that matches volunteers with frail, isolated seniors or adults with disabilities to provide one-on-one socialization. The program provides clients with visits in their own homes; this helps them maintain social contact and integrates them more effectively into their community. The goal of this program is to provide friendly companionship, connect seniors with the community and provide an element of independence.
  • The Medical Escort Program is a support service that matches volunteers with frail, isolated seniors or adults with disabilities to provide escort to medical appointments.
What You Will Do
  • Develop and implement a creative and innovative program plan to achieve the operational goals and targets of the senior’s recreational programs
  • Develop, implement, and evaluate programs which promote wellness through social, recreational, and educational activities.
  • Identify emerging recreational needs and develop new programs to respond to those needs.
  • Coordinate with the broader SALCO team to respond to community needs and ensure existing and new program developments meet these needs.
  • Research, organize, and administer day outings for seniors, with the objective of enhancing their quality of life.
  • Establish positive collaborative relationships with appropriate community agencies, including attending and speaking at community meetings and other functions.
  • Assist with planning and development of program activities for grassroots communities.
  • Liaise with staff from other programs, grassroots groups, volunteers, and community agencies (e.g., hospitals, social workers, doctors, health clinics) to provide program information.
  • Maintain on-going contact with all parties to ensure progress towards targets ensuring adherence to program requirements and perform site visits as required.
  • Resolve conflict situations, including client complaints and disputes.
  • Integrate the principles of anti-racism and community development in programs.
  • Assist with the preparation of regular reports and statistics.
  • Collect and submit program statistics for reporting purposes and internal reviews
  • Ensure program expenditures are consistent with budgetary allocations and in accordance with WoodGreen and funder restrictions.
What You Bring to the Team
  • Bachelor’s degree in social work or a related undergraduate degree required.
  • Minimum two (2) years of experience and proven track record in facilitation of programs in a community setting.
  • Experience in Project Management/Coordination considered a strong asset.
What Will Set You Apart
  • Experience in designing and implementing health training programs with diverse populations.
  • Good understanding and knowledge of seniors’ issues related to social recreational and educational needs.
  • Good understanding of social and wellness issues related to aging.
  • Demonstrated experience and skills in community development within housing buildings.
  • Knowledge and demonstrated skills in program development and management.
  • Able to work independently with minimal supervision.
  • Knowledge of community resources for older adults.
  • Second language is considered an asset.
  • Experience in facilitating community engagement and peer-led initiatives with seniors.
  • Previous work experience in the non-profit social services sector and/or health sector.
  • Solid knowledge of the health and social services sector.
  • Understanding of, and sensitivity to, issues facing immigrants and immigrant communities.
  • Previous work experience with grassroots groups or community groups is an asset.
  • Skills in leading focus group discussions, conducting surveys, and engaging the community.
  • Skills in coordinating activities between multiple agencies.
  • Confidence and initiative to resolve issues, complaints, and conflicts.
  • Excellent interpersonal, verbal, and written communication skills,
  • Patience, compassion, and sensitivity to deal with different cultures and levels of ability.
  • Excellent organizational and time management skills.
  • Strong computer literacy, including database management, report writing, template design, etc.
  • Ability to communicate effectively in one or more languages is an asset.
