About the Opportunity

Reporting to the Community Transitions Health Services Manager, the Wellness Navigator supports citizens of the communities that Community Transitions serves in accessing the services, programs, and/or resources to enable/foster health enhancing behaviors. The Navigator provides guidance and support for clients including providing information about appropriate supports and programs that will meet health and wellness needs and social supports that can assist with daily living and addressing social determinants of health. The Wellness Navigator is a member of an inter-professional team and collaborates with colleagues to determine team goals and objectives.

About You

We would love to hear from you if you have the following:

  • Bachelor's degree in social work or occupational therapy
  • Current and ongoing registration (or eligibility for registration) with the applicable Nova Scotia Professional Regulatory body
  • Minimum three (3) years' experience working in mental health, Primary Health Care setting/community-based team/community setting
  • Current BLS-C certification
  • Experience with individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principle
  • Experience working with people living with a chronic condition
  • Preferred experience in partnership development, stakeholder collaboration, and community development
  • Demonstrated understanding of population health, health promotion strategies, and behaviour change principles; in-depth knowledge of NSH and community-based services
  • Demonstrated person-centered / family-focused, diversity relevant approaches to care
  • Well-developed interpersonal, teamwork, oral and written communication skills
  • Demonstrates flexibility, adaptability and willingness to change
  • Demonstrates innovation in program planning, implementation and evaluation, and quality improvement planning skills
  • Computer skills (MS Office, email and internet)
  • Valid driver's license and access to a reliable vehicle required
  • Competencies in other languages an asset, French preferred

Compensation and Benefits

$ $42.89 hourly

Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
