The Community Health Team (CHT) is a community-based health initiative that focuses on wellness programming, wellness navigation, and building community connections and partnerships. The CHT works in collaboration with other provider groups and services to offer programming and supports. Wellness programming focuses on risk factor management, healthy eating, physical activity, emotional wellness, and parenting (in partnership with the IWK) with behavior change and self-management at the core of all programming.

About the Role:

  • The Wellness Navigator demonstrates flexibility and innovation in supporting citizens of the communities that the CHT serves in accessing the services, programs, and/or resources that can support health-enhancing behaviors.
  • The incumbent provides guidance and support for clients, including providing information about appropriate supports and programs that will meet health and wellness needs and social supports that can assist with daily living.
  • The Wellness Navigator is team-focused and will collaborate with the CHT staff, family physicians, other healthcare providers, and community groups to maintain awareness of existing supports within the community and in the surrounding areas through a community development, population health, and collaborative approach.
  • The incumbent works collaboratively with the team and partners to develop, implement, and evaluate the community-based wellness navigation model that guides their work.
  • The Wellness Navigator is a member of an inter-professional team and collaborates with colleagues to determine team goals and objectives, participates in the evaluation of team effectiveness, solves problems, and shares responsibilities.

About You:

  • We are seeking a candidate with a Bachelor's degree from an accredited institution in one of the following healthcare disciplines: Occupational Therapy, Social Work, Registered Nurse, Recreation Therapy.
  • Registration with the respective professional regulatory association is required.
  • The successful candidate will have a minimum of 3 years' experience, including clinical practice related to their discipline, working in a Primary Health Care setting/community-based team/community setting, and a minimum of one year of recent mental health experience.
  • The candidate will have facilitation/education skills utilizing adult education, self-management supports, and motivational enhancement principles (both individual and group settings).
  • Experience working with people living with a chronic condition(s) is required.
  • Experience in partnership development, stakeholder collaboration, and community development is an asset.
  • Understanding of population health, health promotion strategies, and behavior change principles is required.
  • In-depth knowledge of local health authority and community-based services is required.
  • The candidate will demonstrate person-centered/family-focused, diversity-relevant approaches to care.
  • Well-developed interpersonal, teamwork, oral, and written communication skills are required.
  • The candidate will demonstrate flexibility, adaptability, and willingness to change.
  • The candidate will demonstrate innovation in program planning, implementation, and evaluation, and quality improvement.
  • Computer skills (MS Office, email, and internet) are required.
  • Annual CPR-AED certification and annual WHMIS training and certification are required.
  • A valid driver's license and daily access to a reliable car for travel for work are required.
  • Competencies in other languages are preferred, with French being an asset.
