About the Opportunity

We are seeking a highly skilled and compassionate Wellness Navigator to join our Community Health Team (CHT) at the Nova Scotia Health Authority. The successful candidate will be responsible for supporting citizens in accessing health-enhancing behaviors and social supports that can assist with daily living.

The Wellness Navigator will work collaboratively with the CHT staff, family physicians, other healthcare providers, and community groups to maintain awareness of existing supports within the community and surrounding areas. This individual will also develop, implement, and evaluate the community-based wellness navigation model that guides their work.

About You
  • Bachelor's degree required from an accredited institution in one of the following healthcare disciplines: Occupational Therapy, Social Work, Registered Nurse, Recreation Therapy
  • Registration with respective professional regulatory association required
  • Minimum of 3 years' experience including clinical practice related to discipline, working in Primary Health Care setting/community-based team/community setting, and minimum one-year recent mental health experience
  • Facilitation/education skills utilizing adult education, self-management supports, and motivational enhancement principles (both individual and group settings)
  • Experience working with people living with a chronic condition(s)
  • Experience in partnership development, stakeholder collaboration, and community development
  • Understanding of population health, health promotion strategies, and behavior change principles
  • In-depth knowledge of local health authority and community-based services
  • Demonstrates person-centered/family-focused, diversity relevant approaches to care
  • Well-developed interpersonal, teamwork, oral and written communication skills
  • Demonstrates flexibility, adaptability, and willingness to change
  • Demonstrates innovation in program planning, implementation, and evaluation, and quality improvement
  • Computer skills (MS Office, email, and internet)
  • Annual CPR-AED certification and annual WHMIS training and certification
  • Valid driver's license and daily access to a reliable car for travel for work
  • Competencies in other languages preferred, French an asset
Hours
  • Permanent Full-time position; 75 hours bi-weekly
  • Shifts may include some evenings, weekends, and holidays
Compensation and Incentives

$42.89 Hourly
