About the Opportunity

We are seeking a highly skilled and compassionate Community Outreach Worker to join our team at Nova Scotia Health and IWK Health. As a key member of our mental health and addictions team, you will play a vital role in providing clinical services to adults and their families affected by mental illness and/or substance use or gambling disorder.

The successful candidate will work in both clinic and community settings, providing a range of services including screening, assessment, brief intervention, case management/navigation, individual and group treatment. You will be responsible for navigating, advocating, and coordinating resources and services within Mental Health and Addictions and the community.

We are looking for a candidate with a strong background in social work, preferably with a Bachelor of Social Work degree, and registration or eligibility for registration with a professional regulatory body. You will have a minimum of one year of post-BSW clinical experience in Mental Health and Addictions, with a focus on harm reduction, community development, and psychosocial rehabilitation.

The ideal candidate will possess excellent assessment skills, the ability to implement an evidence-based plan of care, and a working knowledge of the DSM. You will also have experience in offering brief, supportive intervention strategies focusing on harm reduction and recovery, as well as the ability to navigate and de-escalate situations and offer crisis support.

Key Responsibilities:

  • Provide clinical services to adults and their families affected by mental illness and/or substance use or gambling disorder
  • Work in both clinic and community settings, providing a range of services including screening, assessment, brief intervention, case management/navigation, individual and group treatment

  • Develop and implement evidence-based plans of care
  • Collaborate with multidisciplinary teams to provide comprehensive care
  • Provide education and support to patients and their families

Requirements:

  • Bachelor of Social Work degree
  • Registration or eligibility for registration with a professional regulatory body
  • Minimum of one year of post-BSW clinical experience in Mental Health and Addictions
  • Excellent assessment skills and ability to implement evidence-based plans of care
  • Working knowledge of the DSM
  • Experience in offering brief, supportive intervention strategies focusing on harm reduction and recovery
