Home and Community Care Support Services South West is seeking a Patient Safety and Risk Advisor (IPAC)

Opportunity Summary:

Reporting to the Manager, Quality and Risk, the Patient Safety and Risk Advisor (IPAC) is responsible for leading and supporting system-wide and organizational efforts to ensure patient safety and Infection Prevention and Control (IPAC) programs. Specifically, this role focuses on ensuring that patient care is provided in a safe manner for patients and care providers, and that professional and evidence-based practices are utilized by the organization in a patient-centred manner. This role will utilize an IPAC lens for key deliverables, which includes working in partnership with health service providers and the organization's contracted service provider organizations to developed shared approaches and strategies for patient safety. Leadership in key areas of enterprise clinical risk management, patient safety, ethical decision making, patient relations, and emergency management will be essential in this role. This is a temporary full time, Eighteen (18) month opportunity.

The Patient Safety and Risk Advisor (IPAC) will embody Home and Community Care Support Services ("HCCSS") mission, vision and values and apply quadruple aim (enhancing patient experience, enhancing provider/staff experience, improving value and improving populations health) to support continuous quality improvement in daily work. The role will utilize quality improvement tools to drive excellence in care and service delivery and to create a culture of continuous quality improvement. Currently operating in a "work-from-home" model with the potential for a partial or full return to the office, this opportunity is open to the following locations within the South West region: Woodstock, St Thomas, London, Stratford, Seaforth, or Owen Sound.

What will you do?

Key responsibilities include:

  • Leads the development, management and oversight of the organization's Infection Prevention and Control
  • framework, activities and processes, including management of HCCSS IPAC related policies and procedures
  • Supports HCCSS IPAC email inbox to subsequently support staff and clients for consults on IPAC related issues
  • Supports all coaching for patient care and IPAC practices across HCCSS home care teams and SPO
  • Supports the development and implementation the organization's patient safety goals through projects and initiatives, in accordance with strategic plan of the organization, provincial health legislation and regulations
  • Supports the development of a patient safety culture within the HCCSS that promotes safety in reporting and focuses on learning and improvement.
  • Supports the planning, design, implementation, evaluation and documentation of the organization's patient safety activities, including Patient Safety Risk Assessments and Patient Contingency Plans.
  • Leads the development of education materials and information toolkits for staff and service providers related to patient safety.
  • In partnership with other quality team advisors, responsible for leading, planning, directing and coordinating individual improvement initiatives focused on meeting quality and patient safety best practices and standards.
  • Supports the review, analysis and debrief of key patient safety events (adverse events) to understand root cause and to implement improvement activities.
  • Supports the Patient Relations Advisors within the organization in managing and responding to clinical risk events.
  • Consults on cases and where necessary, assists staff to handle and resolve patient safety issues and complaints in a timely and effective manner and involves Patient Care Managers as appropriate when required for coaching and support of staff.
  • Executes quality and risk management initiatives at the patient, system and sub-region level to identify challenges and solutions to improve health system outcomes.
  • Supports the Manager, Quality and Risk in communicating with and advising the Leadership team on matters related to patient safety planning, outcomes, issues management, IPAC and other requirements

What must you have?

  • University or College undergraduate degree in nursing, occupation therapy, physiotherapy or other with an affiliated governing regulatory body;
  • Regulated Health Care Professional;
  • Current Certification in Infection Control (CIC).
  • Greater than 2 years of related experience in infection prevention and control best practice within a healthcare setting. Experience in patient safety, quality improvement, performance management, and risk management in a healthcare environment preferred.
  • Demonstrated change management, facilitation & coaching skills.
  • Communication and inter-personal skills, to work effectively and develop trust with internal and external colleagues, and to liaise with the Ministry of Health and Long-Term Care and other provincial partners. Expert knowledge of Ontario health system, and trends within and across sectors.
  • In-depth knowledge of:
    • Patient safety principles, methodologies, and statistical and analytical processes and techniques;
    • In depth knowledge of IPAC principles and best practices
    • Quality Improvement tools, techniques, research, theories, trends, and measures.
    • Local health issues, priorities and needs while recognizing the broader trends in health care policy and system development.
  • Strong knowledge of qualitative and quantitative measures, evaluation, and utilization measurement
  • Current knowledge of:
    • Legislation relevant to patient care and healthcare in Ontario (e.g. ECFFA etc.);
    • Accreditation Canada standards.
  • Good understanding of indicator development with demonstrated ability to introduce and manage change, new concepts, best practices, and skills development.
  • Ability to use a computer in a Windows-based environment.

What would give you an advantage?

  • Experience and/or training in IPAC preferred.
  • Ability to speak French or another second language.
